Performance Work Statement (PWS) Puerto Rico School Calendar 2015-2016 HEVAS6-15-T-0042 Physical Therapy Solicitation The Department of Defense Elementary and Secondary Schools (DDESS) intends to award a contract using simplified acquisition procedures for Physical Therapy Services in support of the Special Education Program located at 201 Arch Road, Aguadilla, Puerto Rico 00603. Please view combined Synopsis/Solicitation (number HEVAS6-15-T-0042) and Performance Work Statement (PWS) attached. DDESS is a federal government agency that is tax exempt and qualifies for academic/educational pricing. This is a total set aside for Small Business. Only quotes submitted by Small Businesses will be accepted by the Government. Any quote that is submitted by a contractor that is not Small Business will not be considered for award. FSC Code: Q518, Medical Services NAICS: 621340, Offices of Physical, Occupational, and Speech Therapists and Audiologists.
