Revised SOW 1996 Geo-Technical Evaluation Report Geo-Technical Evaluation Report Specifications Structure Repair Drawings Bond Form SF 1442 Representation and Certifications Wage Determination LPTA Solicitation Agreement GENERAL SERVICES ADMINISTRATION (GSA) PUBLIC BUILDING SERVICE (PBS)/REGION 04 - SOUTHEAST SUNBELT REGION ACQUISITION DIVISION/PROGRAM SUPPORT/CAPITAL PROJECTS TEAM (4PQAC) SOLICITATION NOTICE: GS-04P-15-BV-C-0006 This solicitation was posted on June 01, 2015 but removed in error. This is not a new posting but a reposting of the old solicitation with a new extended proposal due date of July 23, 2015. INTERNAL REVENUE SERVICE (IRS) ATLANTA CUSTOMER SERVICE CENTER PARKING DECK REPAIR PROJECT This procurement is advertised as a 100 % total small business set-a-side in accordance with the Federal Acquisition Regulation (FAR 19.5). The GSA is in search of a qualified Small Business FIRM for the following Construction Project summarized below: The requirements of this project are to REPAIR the IRS Atlanta Customer Service Center Parking (Deck) Garage Complex located at 2385 CHAMBLE TUCKER ROAD, CHAMBLEE, GA 30341. The existing parking structure is in need of major repairs to resolve structure, serviceability and strength issues. The issues are outlined in the 2010 Engineering Study and the 2011 Destructive Testing and Analysis report. This repair description and specification information will be included in the Statement of Work(SOW) for this project. The goal of the project is to allow for all parking spaces to be utilized by strengthening the existing elevated slabs and the supporting columns. Repair of all concrete/CMU cracks and spalls will be required to return the deck to serviceability and resolve the strength issues. The repairs should also extend the service life of the structure and resolve parking limitations for the IRS employees and clients. Contractor/Subcontractor employees will need to be in compliance with HPDS12 requirements for this project. The NAICS code for this project is 238110, Poured Concrete Foundation and Structure Contractors applies to this project with size standard of $15.0. The estimated cost range of the entire contract including options has an order magnitude in the amounts between $4,000,000.00 and $5,000,000.00 dollars. Please note that under FAR 52.219-14(3) the contractor shall perform at least 15% of the cost of the contract, not including the cost of materials with its own employees. The contract duration is estimated at 730 calendar days. The selection process will utilize the Lowest Priced Technically Acceptable (LPTA) Proposal in accordance with Federal Acquisition Regulations (FAR19.5) and outlined in the solicitation. The GSA Contracting Officer for this project is Joseph Ardoin. The GSA Project Manager for this project is Brandy Nabors. The Request for Proposal with detailed information regarding the selection criteria and requirements for this procurement is attached. All questions must be sent in writing to joseph.ardoin@gsa.gov. Any questions submitted by phone will not be addressed. The Site Visit and Pre-proposal conference will be combined and conducted on June 17, 2015. All interested vendors must submit their team access request as outlined in the solicitation to brandy.nabors@gsa.gov. Bid, Performance and Payment bonds will be required in the total amount of the proposed cost and/or contract award price: Minimum Bonding Capacity estimated as $5M per contract. PROJECT DESCRIPTION The intent of this project is to renovate the IRS Parking Garage Complex located in Chamblee, GA. The renovations include renovation of existing Parking Structure. Bid, Performance and Payment bonds will be required in the total amount of the proposed cost and/or contract award price: Minimum Bonding Capacity estimated as $10M per contract. CAPABILITY STATEMENT The following statements will apprise the General Services Administration of prospective Contractor capabilities required by the above project description. Please provide your response to the following and limit your submission to 3 pages. 1) Contractor's name, address, primary point of contact, phone number and email address. 2) Contractor's DUNS and CAGE code 3) Contractor's capability to perform a project of this magnitude and complexity, to include comparable work performed within the past 5 -7 years. Provide brief descriptions of at least 3 Design/Build Construction projects that were performed in an occupied building, include project name, contract number (if Federal Government), timeliness of performance, customer satisfaction, and dollar value of the project. For each example describe if the project was self performed or a team effort. 4) Contractor's small business classification: Service Disabled Veteran Owned Small Business; Woman Owned Small Business or HUBzone. 5) Contractor's Joint Venture or teaming information if applicable - existing and potential - and the same information for the mentor company as described, above, for small businesses. 6) Contractor's Capability in the form of a letter from Surety, per contract and with aggregate amounts.
