DAVIS-BACON WAGE DETERMINATION EXPERIENCE QUESTIONNAIRE SF1442 FROM SOLICITATION TO COMPLETE AND RETURN SOLICITATION A&E EVALUATION REPORT PLANS TECHNICAL SPECIFICATIONS DIVISION 1 SPECIFICATIONS A PRE-BID MEETING IS SCHEDULED ON THURSDAY, JULY 23rd, 2015 AT 10:00 AM Eastern. LOCATION IS THE OTTAWA NATIONAL FOREST KENTON RD ADMIN OFFICE, 4810 EAST M28, KENTON, MICHIGAN. DIRECTIONS: From the intersection of US Highway 45 & M-28 in Bruce Crossing, Michigan, travel 16.5+/- easterly along M-28 to the Ottawa National Forest, Kenton Ranger District Administrative Office on the north side of M-28. The Lake States Acquisition Team is announcing an opportunity to bid on a project for Kenton Ranger District Administrative Office Boiler / Heating System Upgrade Project. Located at 4810 E. M-28, Kenton, Michigan 49967, Houghton County. This acquisition is advertised as a total small business set-aside using Simplified Acquisition Procedures. In accordance with FAR Part 36.204(b), the estimated magnitude of construction is between $25,000 and $100,000. The applicable North American Industry Classification System (NAICS) Code is 238220. The Small Business Administration size standard is $15 million. Contractors are required to be actively registered with the System for Award Management (SAM) database and completed their online representations and certifications. Davis-Bacon wage rates will apply to this contract. Payment protection is required on bids exceeding $30,000. No reimbursement will be made for any costs associated with providing information in response to this solicitation, amendments, or any follow up information requests. No hard copies of the solicitation, supporting documents and applicable amendments, if any, will be provided. To receive the entire solicitation package and any applicable amendments, prospective offerors must download them from the FedBizOpps website. Contract estimated completion date is on or about October 2, 2015. This project involves the replacement, retrofit, alteration and/or addition of parts, components of the existing Boiler/heating System with new, energy efficient items, parts, components; this work includes, but is not limited to: Removing and disposing of existing boiler, boiler vent, outdoor air reset, pumps, expansion tank, and air scoop ; Installing specified energy efficient boiler, pumps, expansion tank, piping, associated accessories; Removing existing pipe insulation and replacing with new pipe insulation per plans & specs; Labeling new & existing heating system piping; Rezoning of rooms per plans & specs; Replacing & Relocating thermostats; Draining & flushing entire hydronic system (which includes notifying FS contract administrators of the presence of glycol in the existing sys tem, and proper disposal & documentation of glycol/contaminated fluid if present); Testing, adjusting, and balancing of upgraded boiler/heating system; Demonstration training FS personnel in operating (i.e. start-up, shut-down, normal operation, trouble shooting, etc.) the revised/upgraded system; preparing and providing O&M manuals for the revised/upgraded system to the Ottawa National Forest All invoices must be submitted via the electronic Invoice Processing Platform (IPP).
