Description
Provisions and Clauses Statement of Work This is a solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in the notice. This solicitation, W912K6-15-Q-0059, is being issued as a Request for Quote (RFQ); quotations are being requested. A draft copy of this RFQ was handed out at the mandatory site visit conducted 17 July 2015 at 10:00AM per the Pre-solicitation notice. After the site visit, all documentation will be provided via FBO only. This RFQ constitutes a solicitation and incorporates provisions and clauses in effect through FAC 2015-83 (effective 2 Jul 15). This acquisition is a total small business set aside. The NAICS is 238390. The small business size standard is $15,000,000.00. The contract type will be a firm fixed price purchase order. In accordance with FAR 52.212-2 Evaluation-Commercial Items, evaluation will be based on lowest price, technically acceptable. In order to be deemed technically acceptable, all items shall conform to the statement of work and equal products must be identified to be reviewed. Vendor shall submit a delivery schedule that will be reviewed for reasonableness and the majority of the items shall conform to environmental concerns and meet a minimum of LEED Silver certification. Quotes that clearly meet the requirement will be deemed acceptable. Quotes that do not clearly meet the requirements will not be acceptable and will not continue to be evaluated. Your quotes must be valid for 90 days and will consist of the items in the statement of work. Quotes will be submitted using the following contract line items (CLINS): 0001 Base: Furnish and install all furniture and equipment for rooms numbered 102, 104, 111, 210, 217, 218, 219, 221, 210 A, 204, 220. Furniture $ ________________________ Installation $ ________________________ 0002 Option: Furnish and install all furniture and equipment for rooms numbered 211, 212, 216A, 204A, 204B, 204C, 204D, 119, 120, 119A, 128, 103, 110, 102, 111, 205, 206, 204, 220, 123. Option may be exercised not later than 90 calendar days from receipt of quote. Furniture $ ________________________ Installation $ ________________________ 0003 Option: Furnish and install all furniture and equipment for rooms number 102A, 102B, 111 A, 111 B, 204, 220, 213, 115 A, 202, 203, 216, 35 gallon trash cans, recycle bins. Option may be exercised not later than 90 calendar days from receipt of quote. Furniture $ ________________________ Installation $ ________________________ 0004 Option: Furnish and install all furniture and equipment for room number 100. Option may be exercised not later than 90 calendar days from receipt of quote. Furniture $ ________________________ Installation $ ________________________ 0005 Option: Furnish and install all fitness equipment that that are called for in room 109. Option may be exercised not later than 90 calendar days from receipt of quote. Option may be exercised not later than 90 calendar days from receipt of quote. Furniture $ ________________________ Installation $ ________________________ Requests for information must be submitted via email not later than 11:59PM 24 July 2015. Answers and clarifications will be provided as an amendment to the solicitation. Answers and clarifications are considered to be addendums to the Statement of Work and will be included in the contract. See attached documents for provision and clauses and the statement of work. Quotes must be submitted not later than 11:00AM on Tuesday, August 18, 2015. Proposal submission via USPS, Commercial Carriers or hand delivered to: USPFO for Maryland, Purchasing and Contracting Division 301 Old Bay Lane Havre de Grace, Maryland 21078-4003 MARKED FOR: P&C, DRC Furniture, W912K-15-Q-0059 Vendor shall submit their proposal in two (2) volumes. Volume one (1) shall be your technical capability documentation not to exceed twenty-five (25) pages. Volume two (2) shall be your proposal. Submit 4 copies of Volume 1 and 2 copies of Volume 2. The intent of the Government is to award one contract for all items but it is the discretion of the contracting officer to determine if multiple awards will be allowed. Vendors doing business with the Maryland Army National Guard must be registered or be willing to register with System for Award Management at www.sam.gov.
