July 20, 2015 MEMORANDUM FOR: Service-Disabled Veteran-Owned Small Businesses (SDVOSB) FROM:Orlando Veterans Affairs Medical Center Contracting Office (Room 210) 2500 Lake Mont Avenue Orlando, FL 32814-6102 SUBJECT: Request for Quotation (RFQ) VA248-15-Q-1674 Preventive Maintenance Inspection of Emergency Generators and Fuel Testing 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and FAR 13.5, Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All responsible offers may submit a quotation that is timely received shall be considered. 2. This a RFQ for the purchase of services to provide PMI of emergency generators, inspections (quarterly and annual) and an annual chemical lab analysis on the diesel fuel utilized by the emergency generators at multiple locations throughout central Florida region. This requirement is solicited as a SDVOSB set-aside under Public Law 109-46, Veterans Benefits, Health Care, and Information Technology Act of 2006. North American Industry Classification System (NAICS) code NAICS 811310 Size Standard $7.5M. 3. The Statement of Work (SOW) included as attachment A includes the Government's objectives for this procurement. Additionally, the following attachments are included and part of this solicitation: a.Applicable Clauses and Provisions, Attachment B b.Service Contract Act (SCA) Wage Determination, Attachment C c.Pricing Information Sheet, Attachment D d.Quotation Submission Instructions and Evaluation Criterion, Attachment E1 thru E2 e.Past Performance Information Questionnaire (PPIQ), Attachment F f.Generator Photos G g.Site Visit, Attachment H 4. By submission of a quotation in response to this RFQ, the Offeror certifies their adherence to all RFQ requirements, including but not limited to the Center for Verification and Evaluation (CVE) registration requirements, terms and conditions of the solicitation, and representations and certifications. Offerors shall clearly identify and provide sufficient rationale detail to justify any exception to the RFQ and/or its related documents. 5. The required performance start date for this RFQ is: August 24, 2015 6. The point of contact for this RFQ is Ms. Rosa I. Rivera, Contract Officer, Orlando VA Medical Center Services Team. This RFQ is not an authorization to begin performance and in no way obligates the Government for any costs incurred by the contractor in regards to this procurement. Offerors shall honor quote no less than 60 days after quote award. The Government reserves the right to cancel, this solicitation, either before or after the stated closing date for receipt of offers. In the event of a RFQ cancellation, the Government has no obligation to reimburse any potential Offeror for any costs incurred. 8. Submit offers electronically in accordance with the RFQ submission instructions as outlined in Attachment E, on or before, August 3, 2015, no later than 11:00 a.m. Eastern Standard Time (EST).
