QUESTIONS & ANSWERS: 1.Need to Clarify for Goose Creek and Myrtle Beach about the Working Supervisor (Monday Through Friday 0800am-4:30PM)? As stated in contract: Normal Work Hours: 4:00pm-12:00 Midnight, Monday through Friday at all locations. In addition, at the Myrtle Beach Primary Care location there will be one working supervisor on duty Monday through Friday, 8:00am-4:30pm and during special cleaning projects. At the Naval Weapon Station C&P and Optometry Buildings, the contractor shall provide lunchtime service of restrooms in both buildings and light cleaning at noon Monday through Friday. Restroom service is considered at a minimum to be thorough general cleaning and replenishing paper products as necessary. Contractors Work Schedule shall take into account extended clinic hours to include Wednesday evening until 6:30pm and Saturday mornings from 8:00am-12:00 noon. This will include use of approximately four exam rooms and may also require additional cleaning of the clinic entrance, check-in/waiting areas, blood drawing lab and restrooms. 2.What is the distance between the sites? Please use map quest to determine the distance based on the address provided in the solicitation. 3.Is one supervisor required at each site, i.e., 3 supervisors during day shift? As stated in contract: Normal Work Hours: 4:00pm-12:00 Midnight, Monday through Friday at all locations. In addition, at the Myrtle Beach Primary Care location there will be one working supervisor on duty Monday through Friday, 8:00am-4:30pm and during special cleaning projects. Monthly charges/ how much service will cost per month 5.Will Saturday work hours require a janitor from 8:00am-12noon or cleaning after closing? Yes 6.Is there any carpet shampooing required under this contract? In the chart for minimum cleaning frequency there is no mention of carpet cleaning, however it is mentioned in Section D paragraph 11. 7.Is the contractor responsible for the purchase of the red bags for infectious waste? No 8.Are there any costs involved in the preparation of red bag waste for shipment at the Myrtle Beach Specialty Care Annex facility? No 9.Are we to assume that we are to provide a working supervisor 40 hours per week for day porter services and daily cleaning at each clinic in addition to our night cleaning crew? Yes. 10.Does the day porter perform the daily cleaning tasks and night crew perform the periodic tasks (Weekly, Monthly and quarterly tasks) see above. A working supervisor is required on day shift. How he/she splits up work is an operational decision, not a contractual one. 11.Saturday Work Schedule: 8:00am-12:00 noon does this mean cleaning is performed after 12:00 noon yes 12. Is there restriction on a completion time no. 13.Is Sundays available to complete periodic tasks if needed - no, assuming the PIV card is issued, security, codes, keys, etc. are all in place? 14.What is the total square footage of the C&P CLINIC BUILDING & OPTOMETRY CLINIC Building it states that the approx. total square footage is 16,926 please clarify total square footage? Myrtle Beach Primary Care Clinic total approximate square footage for is 16,926 with an estimated square footage of carpet area of 500 and an estimated square footage of uncarpeted area of 16, 426. They are a total of 9 bathrooms; 46 sinks; 2 urinals and 9 toilets. They are no showers and no locker rooms. They are 40 exam rooms; 24 office spaces; 4 conference rooms; 2 break rooms; 4 storage rooms and 2 waiting/lobby areas. Total approximate square footage for the C&P Clinic Building is 2,200 and 2,726 for the Optometry Clinic Building. There are no carpet area and an estimated 4,926 square footage of uncarpeted area. They are 4 bathrooms; 11sinks; 2 urinals; 4 toilets; and 1 shower. They are no locker rooms; 14 exam rooms; 3 office spaces; and no conference rooms. They are 2 break rooms; 2 storage rooms and 2 waiting/lobby areas. 15.How many cases of toilet tissue, paper towels, toilet seat covers, antimicrobial hand soap, alcohol foam hand cleaner, and trash can liners are used per month at each clinic. Or do we just charge additional for the quantity used each month? This is operational. All clinic appointment visits, total patients, total staff, etc. were provided in another inquiry. There will NOT be any additional charges made against this contract for supplies. You need to estimate and bid accordingly. 16.What are the specifications of each product aforementioned? Negotiable, aside from soap and sanitizer. Please refer to SOW 17.If awarded the contract, will we be able to keep the existing work force? No 18.Can you provide the existing contract pricing? VA247-14-P-3288 When does the contract expire? 9/30/2015 What is the current contract value? $337,325.52 (5 years) 19.What is the approximately monthly spend on paper products and cleaning supplies? Unknown - contractor provides based on usage. 20. At the Specialty Care Annex the only work done is the collection of Infectious Red Bags daily (Monday - Friday)? Yes. Collecting red bag waste, preparing it for shipment, and keeping records of what was shipped. 21.The solicitation states in Section A that the Myrtle Beach Clinic is 16,926 square feet. Then states in Section B that "Total approximate square footage is 16,926: 2,200 C&P Clinic Building; 2,726 Optometry Clinic Building." Is this correct? Total approximate square footage is 16926: 2,200 C&P; 2,726 Optometry Clinic; 12,000 Myrtle Beach Primary Care. 22.In Section B it states there will be one working supervisor on duty Mon- Fri 8:00am to 4:30pm. At which facility will the supervisor be required to work? Myrtle Beach Clinic 23.Do the facilities in this solicitation require all day janitorial services during working hours? As stated in contract: Normal Work Hours: 4:00pm-12:00 Midnight, Monday through Friday at all locations. As stated in the statement of work - Normal Work Hours: 4:00pm-12:00 Midnight, Monday through Friday at all locations. In addition, at the Myrtle Beach Primary Care location there will be one working supervisor on duty Monday through Friday, 8:00am-4:30pm and during special cleaning projects. 27.What time is the contractor expected to begin and end a shift? As stated in the statement of work - Normal Work Hours: 4:00pm-12:00 Midnight, Monday through Friday at all locations. In addition, at the Myrtle Beach Primary Care location there will be one working supervisor on duty Monday through Friday, 8:00am-4:30pm and during special cleaning projects. 28.Can stripping of floors be done during night or weekend? Yes, at no additional cost to the government. 29.What is the current usage of paper products? Hand towels, toilet tissues etc. This is provided by the contract holder, VA would not have this info. 30.How many workers serve the current contract requirements? 4. We dictate the hours worked not how they are split up. 40. Can you clarify the days and hours needing coverage at the Primary Care Clinic, Specialty Care Annex, C & P Clinic Building and Optometry Clinic Building? As stated in the statement of work - Normal Work Hours: 4:00pm-12:00 Midnight, Monday through Friday at all locations. In addition, at the Myrtle Beach Primary Care location there will be one working supervisor on duty Monday through Friday, 8:00am-4:30pm and during special cleaning projects. 41.For clarification, are you asking for one working supervisor in each location Monday - Friday and a full shift staff from 4pm - 12 midnight Monday - Friday? Supervisor @ MB only. Supervisor to be essentially a day porter and assign projects to night shift where the bulk of work can get done. C&P/Opt are very small and just need a housekeeper. However, it is expected that the contract holder inspects the work and has a quality assurance program, as stated in the contract. 42.On the Cleaning Frequency Chart can you clarify Scrub floors/recoat as needed (5 coats) Monthly and Top scrub and recoat tile floors (5 coats) Quarterly. Are they the same floor? By reading the frequency chart: MONTHLY is under the restroom heading and would apply to any restroom floors that are intended to be waxed. QUARTERLY applies to all other floors (hallways, offices, etc.). BOTH type floors also have "as needed" checked as well - meaning: if traffic or usage or improper daily maintenance dictates more frequent scrubbing and waxing - that is what is required. 43.Are the buildings single story? Yes 44.The solicitation state the normal working hour and that Wednesday has extended hours however, it does not say whether the contractor is required to work after duty hour or during duty hours Mon-Fri? The clinic cannot be cleaned while they are seeing patients. 45.What time is the contractor required to start working on Wednesday at the C&P clinic and the optometry building? What time can the contractor start cleaning, if the hours are extended to 6:30PM? No 49.Would there be any way to schedule another site visit for these places of performance listed in the RFP? No further site visits will be scheduled. Please review the attached floor plans and submit any questions that you may have. All questions regarding this solicitation need to be electronically submitted (email) no later than July 15, 2015 at 5:00 pm EST to Janica Francis-Hunter, NCO 7 Contract Specialist e-mail Janica.Francis@va.gov. The solicitation number must be identified on all submitted questions. No questions received after this date will be answered. 50.Do you have a list of recommended supplies and usage? Soap and hand sanitizer are prescribed by the hospital and are non-negotiable. Cleaning chemicals and consumable paper supplies are negotiable and must be approved prior to first use by submitting a list of supplies and applicable technical data sheets and safety data sheets. 51.Is there a requirement for aseptic cleaning? Yes, it is a clinical setting. Environmental surfaces must be disinfected with an EPA approved germicide. It indicates this in the SOW. 52.Were there any modifications added to the original contract number: VA247-14-P-3288 during the 5 year term for this vender? Yes 53.How much is the current amount being paid with the addition of any modification(s)? $342,365.52 54.What was the monthly payment from last year service? Approximately $5,700 55.Could you confirm the response due date for the project above? 08/01/15 56.Section A- Says there is a specialty care annex that appears to be across the street from the Primary care clinic, in what looks to be a mall/ strip mall? Can you confirm? Yes. It is across the street in a medical office building 57.Section A - The care annex only needs daily pickup of the red bag waste but it does not specify where it is to be delivered, is it safe to assume that it would be at the Primary Care clinic? At the Myrtle Beach Specialty Care Annex, 1101 Johnson Ave, Suite 204, Myrtle Beach, SC 29577 the contractor shall collect infectious "red bag" waste daily and prepare for shipment. Destruction of the infectious waste this contract will be collected at this building. 58.Will the janitorial contract services for the Primary Care also require red bag pickup? Yes. 59.What are the hours and days of the care annex clinic for the red bag pickup? Hours of operation: 8-4:00 Mon-Fri 60.There are what appear to be attachments inside the original document, but there is not an actual file to open. They are only pictures of the file icons. Could those attachments be uploaded on to FBO? Please see attachment. 61.What type of paper towel and toilet paper dispensers are at each of the locations? White Folded Towel Dispenser & Center Pull Towel Dispenser 62.What type of hand soap dispensers are at each location? The soap dispensers are Provon -handsfree. Soap and hand sanitizer are prescribed by the hospital and are non-negotiable. Cleaning chemicals and consumable paper supplies are negotiable and must be approved prior to first use by submitting a list of supplies and applicable technical data sheets and safety data sheets. 63. Is there a site visit scheduled? If not, could I schedule one? Site Visit for C&P Clinic Building & Optometry Clinic Building was be held on Tuesday, July 7th, 2015 at 11:00 a.m. Site Visit for Myrtle Beach Primary Care was be held on Thursday, July 9th, 2015 at 11:00 a.m. No future sight visits will be scheduled.
