- Cover Letter The Federal Bureau of Prisons, Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based out-patient Substance Abuse and Mental Health Services in Beaumont, Texas. Location of services should be within a 10 miles of 1310 Pennsylvania Avenue, Beaumont, Texas 77701. Performance periods under this contract will be for a one-year base period to begin on November 01, 2015 with four (4) one-year option renewal periods with service ending (if all options are renewed) on October 31, 2020. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor. Base Year: Estimated Number of Inmates = 30 Substance Abuse Treatment Services: Intake Assessments & Report (one per authorized inmate) for 30 inmates = 30 assessments Individual Counseling Sessions (30 minutes = 1 unit) 20 units (per inmate) x 30 = 720 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 30 = 2160 units Family Counseling (30 minutes = 1 unit) 12 units (per inmate) x 10 inmates = 120 units Mental Health Treatment Services: Mental Health Intake Assessment & Report ( five per inmate) for 10 inmates = 50 units Psychiatric Evaluation & Report = 8 report per authorized inmate x 4 inmates = 32 reports Mental Health Individual Counseling (30 minutes =1 unit) (24 units per inmate) x for 10 inmates = 240 units Mental Health Group Counseling (30 minutes =1 unit) (20 units per inmate) x 20 inmates = 400 units Mental Health Medication Monitoring ( four visits per inmate) for 10 inmates = 40 monitoring visits Transportation (50 trips per inmate) x 25 Prisons Community Treatment Services most recent Statement of Work dated April 2011- No Masters Required, Revision 01, 2013, PSB. The request for quote will be available on or after July 16, 2015 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted. The Bureau anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
