Description
THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is NOT a solicitation for proposals, proposal abstracts or quotes. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources within the specific areas to perform this potential requirement. The Department of Veterans Affairs Network Contracting Office 19 (NCO 19) Health Care for Homeless Veterans (HCHV) is seeking providers that have experience and a proven record of successfully working with residents recovering from co-morbid conditions such as serious mental illnesses, substance abuse disorders and other serious medical conditions. The VA anticipates multiple indefinite-delivery, indefinite quantity contracts to satisfy this requirement. Services are sought for existing facilities in counties within the jurisdiction of the NCO 19 Rocky Mountain Network to include the following medical centers: Eastern Colorado Health Care System, Denver Colorado: VA Medical Ctr., Grand Junction, CO; the Salt Lake City Health Care System, Salt Lake City Utah; VA Montana Health Care System, for Harrison Montana VA Medical Ctr., Sheridan, WY and the VA Medical Ctr., Cheyenne, WY. The period of performance for this this proposed contract is a Base Year, with provisions in the contract to extend services, at the discretion of the Government, for 4 one-year option period. Services will include room and board, including at least 3 nutritionally adequate meals per day, 7 days a week, laundry facilities for residents to do their own laundry or to have laundry done, therapeutic and rehabilitation services determined to be needed by the individual patient in a plan developed by the contractor with input from the patient and the HCHV Case Manager. The NAICS Code for this solicitation is 624221, with a small business sites standard of $11 million. No formal solicitation document exists at this time; however the solicitation shall be released and posted to www.fbo.gov on or about August 31, 2015.
