- SOURCE SOUGHT THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. IT IS NEITHER A SOLICITATION ANNOUNCEMENT NOR A REQUEST FOR PROPOSALS OR QUOTES AND DOES NOT OBLIGATE THE GOVERNMENT TO AWARD A CONTRACT. REQUESTS FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. RESPONSES TO THIS SOURCES SOUGHT MUST BE IN WRITING. THE PURPOSE OF THIS SOURCES SOUGHT ANNOUNCEMENT IS FOR MARKET RESEARCH TO MAKE APPROPRIATE ACQUISITION DECISIONS AND TO GAIN KNOWLEDGE OF POTENTIAL QUALIFIED NURSING HOMES IN DUTCHESS, PUTNAM AND/OR ORANGE COUNTY WITH VENTILATOR CARE INTERESTED AND CAPABLE OF PERFORMING THE WORK. DOCUMENTATION OF TECHNICAL EXPERTISE MUST BE PRESENTED IN SUFFICIENT DETAIL FOR THE GOVERNMENT TO DETERMINE THAT YOUR COMPANY POSSESSES THE NECESSARY FUNCTIONAL AREA EXPERTISE AND EXPERIENCE TO COMPETE FOR THIS ACQUISITION. RESPONSES TO THIS NOTICE SHALL INCLUDE THE FOLLOWING: (A) COMPANY NAME B) ADDRESS (C) POINT OF CONTACT (D) PHONE, FAX, AND EMAIL (E) DUNS NUMBER (F) CAGE CODE (G)TAX ID NUMBER AND (H) MUST PROVIDE A CAPABILITY STATEMENT THAT ADDRESSES THE ORGANIZATIONS QUALIFICATIONS AND ABILITY TO PERFORM AS A CONTRACTOR FOR THE WORK DESCRIBED BELOW. SYNOPSIS: THE VA NEW HUDSONVALLEY HEALTHCARE SYSTEM IS LOOKING FOR A CONTRACTOR TO PROVIDE NURSING HOME SERVICES WITH VENTILATOR CARE AND TRAUMATIC BRAIN INJURY IN DUTCHESS, PUTNAM AND/OR ORANGE COUNTY. THE SCOPE OF SERVICE INCLUDES BUT IS NOT LIMITED TO: 1. The Contracted Nursing Home shall ensure that care meets the health needs and promotes the maximum wellbeing of VA registrants. Contracted Nursing Home will provide social and recreational activities for VA beneficiaries. The care provided will include: nursing, social work services, ventilator care, occupational or physical therapy, assistance with activities of daily living, activity therapy, case management, and nutrition service including meals. These services will be at the same frequency as that provided to other registrants at the Contracted Nursing Home receiving the same or comparable level of care. Registrants will receive individualized care plans tailored to meet the needs of each. The per diem rate(s) established in this contract will include the cost of care, and other special services authorized by VA, unless otherwise specifically accepted. The rate established will include the services listed and will also include all services or supplies normally provided to other Contracted Nursing Home patients by the facility without extra charge. 2. Full attention shall be given to motivating and educating registrants to achieve and maintain independence in the activities of daily living, to the maximum extent possible. Every effort shall be made to keep registrants ambulatory and to achieve an optimal level of self-care. 2.1. Nursing Services: At least one RN must be on duty at all times whenever Veterans are present, for assessment, participation in treatment planning, and supervision of other nursing staff. Additional nursing staff may include a licensed practical nurse, nursing assistant, or health technician who provides assistance in daily program activities. Assistance for daily living including bathing, feeding, toileting, ambulation and transferring will be provided. 2.2. Social Services: There will be sufficient social service staff available to provide a social assessment and counseling to registrants and their caregivers. Coordination of registrants Contracted Nursing Home program with his/her outside social and health support systems will be provided. Discharge planning and continuity of care beyond Contracted Nursing Home will be given to each registrant. Records will include a treatment plan, periodic evaluations, progress notes and other clinical and social information pertinent to the registrants care. 2.3. Ventilator Care: Veterans requiring the use of a ventilator will only be placed in a CNH-trained and equipped for those types of specialty patients. The CNH must be inspected and approved by the VA prior to any placements in the ventilator-designated room. Rates for ventilator care will be detailed in the rate table and will be an all-inclusive per diem rate (including ventilator and supplies). 2.4. Rehabilitative Services: Maintenance rehabilitation services will be provided as needed with consultation of a qualified therapist (Physical and/or Occupational Therapist). 2.5. Nutrition Services: Nutrition services will be provided with the consultation of a registered dietitian. Nutritional services include counseling, regular meals and special diets. Nutrition services must be provided daily, including regular meals, snacks, and special diets. At a minimum, a registered dietitian must be available by consultation for assessment and education 2.6. Recreation Services: Therapeutic recreational services will be provided to meet the physical and social needs of the registrants and will be an integral part of the individual treatment program. 3. It is agreed that the Contracted Nursing Home will notify the authorizing VA Hospital immediately if an emergency requiring hospitalization occurs. In the event a VA beneficiary receiving Contracted Nursing Home care under this contract dies, the Contracted Nursing Home will promptly notify the VA office authorizing admission and immediately assemble, inventory, and safeguard the patient's personal effects pending further guidance by VA. 4. The authorization agreement, VA Form for the affected patient will terminate whenever a VA patient is hospitalized or placed in a higher level of care for 15 calendar days or more, whether in a VA or in a non-Federal facility. A new authorization agreement will be required at the time the patient returns. 5. It is agreed that VA will have the right to on-site reviews of the Contracted Nursing Home and all appurtenances by authorized VA representative(s) to ensure that acceptable standards are maintained and that the necessary care to maintain the well-being of the patient is rendered. 6. The Contracted Nursing Home shall accept referral of and shall provide all services specified in this contract for any person determined eligible by the VA under Secretary for Health or his/her designee, regardless of the race, color, religion, sex, or national origin of the person for whom such services are ordered. In addition, the Contracted Nursing Home warrants that subcontracting will not be resorted to as a means of circumventing this provision. 7. It is agreed that duly authorized representatives of VA will provide follow up supervision visits to veterans placed to assure the continuity of care and to assist in the veteran's transition back to the community. It is understood that these visits do not substitute for nor relieve the Contracted Nursing Home in any way of the responsibility for the daily care and treatment of the veteran. 8. The Contracted Nursing Home shall obtain appropriate insurance coverage to include but not to be limited to personal and professional liability. 9. Invoices shall be submitted promptly to the VA medical Center making the placement, by the 15th calendar day following the end of the month in which services were rendered. All invoices must include the full name and address of the Contracted Nursing Home provider and shall reflect the VA beneficiary's name, social security number; number of days billed and rate(s). Failure to include this information may result in delayed payments. Absences of the veteran from the Contracted Nursing Home will not be reimbursed. 10. Each monthly invoice submitted shall be accompanied by a report. 11. A record shall be maintained for each registrant. All clinical records concerning the registrants care in the Contracted Nursing Home will be readily accessible to VA. Upon discharge or death of the registrant, medical records will be retained by the Contracted Nursing Home for a period of at least three years following termination of care. Patient records will be maintained in conformance with the Privacy Act of 1974 (5 U.S.C. 552a). 12. The VA, at its sole option, will monitor the professional care and administrative management of services provided to VA beneficiaries under this agreement, through one or any combination of the following methods; reviews of state agencies reports, on-site review of the Contracted Nursing Home by VA staff, and/or on-site monitoring of VA registrants. It is agreed that the Contracted Nursing Home shall provide VA with copies of all state agency reports when requested, and cooperate fully with VA's quality improvement-quality assurance program functions relating to this agreement, including VA's on-site review and monitoring. The VA contracting Officer shall make all final determinations as to the contractor's reasonable cooperation with VA and compliance with these requirements. It is understood that certain internal quality indicator/quality assurance information is not available for VA review. Current quality measure and current individual MDS information will be made available. 13. ONGOING MONITORING AND FOLLOW-UP VISITS IN CNHs 13.1 Prior to placement of the veteran in a nursing home, a plan is to be developed for follow-up visits needed from the VAHCF. This plan is to be developed by social work and/or nursing staff involved in discharge and placement, in consultation with the referring bed service or clinic. The plan needs to delineate, on an individual patient basis, the particular needs and services to be provided to the patient. 13.2. Every VA patient under contract in a nursing home must be visited by a social worker or registered nurse at least every 30 days, except as noted. Social workers and nurses will alternate monthly visits, unless otherwise indicated by the patient's visit plan. Other professional disciplines need to make follow-up visits when indicated by the patient's discharge plan, or upon recommendations from the CNH Review Team. NOTE: It is important to emphasize the individual basis of this plan. When visits become routine, there is a danger that the focus will be lost and that quality will suffer. 14. ANNUAL REVIEW OF CONTRACTED NURSING HOME 14.1. For the Contracted Nursing Home, the review process is completed and documented every 12 months by the VA Nursing Home Review Team, and no more than 90 days prior to expiration of the contract. If a contract is canceled and renegotiated during the year for the purpose of establishing a new per diem rate or for an ownership change, it is not necessary to conduct another review as long as the review has been conducted within the required 12-month time limit. 14.2. The Nursing Home Team repeats the process described in Paragraph 14.1 for annual reviews. In addition, the team must review the findings of the ongoing monitoring visits to the nursing home, as described in Paragraph 14.1. NOTE: Triennial site surveys for LSC compliance will be conducted, unless otherwise indicated by the review process. 14.3. The VA Nursing Home Review Team must document the findings and recommendations on each review and follow-up review conducted. 14.4. Based on the preceding review processes, the VA Nursing Home Review Team through the Nursing Home Coordinator, makes recommendations to the Contracting Officer on the disposition of the contract renewal. 14.5. The VA Nursing Home Review Team through the Oversight Committees, provides documentation on its findings and the Nursing Home disposition or the Certification Report.
