Description

Combined Synopsis and Solicitation i.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement, and its attached solicitation, constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. ii.Department of Veterans Affairs, Network Contracting Office (NCO 22), 4811 Airport Plaza Drive, Suite 600, Long Beach, CA, 90815 is issuing solicitation VA262-15-Q-1000 to request proposals from offerors that can provide genotyping services in accordance with the terms and conditions of the statement of work contained in the attached solicitation. iii.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80 published in January 29, 2015. iv.The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). v.A list of contract line item number(s) and items, quantities, and option years appear in Section B.3- Price/Cost Schedule of the attached solicitation. vi.A Description of requirements for the items to be acquired appears in section B.2 Statement of Work of the attached solicitation. vii.Date and place of delivery and acceptance and FOB point appear in the solicitation cover sheet (SF 1449) viii.Addendum to provision 52.212-1, Instructions to Offerors - Commercial applies to this acquisition. ix.The specific evaluation criteria to evaluate offers resulting from this solicitation appear at provision 52.212-2, Evaluation -- Commercial Items. x.Offerors are advice to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. xi.Clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The additional FAR clauses cited in the clause are applicable to the acquisition. xii.See Statement of Work regarding any additional contract requirement(s) or terms and conditions. xiii.The date, time and place offers are due, and the name and contact information of the individual to contact for information regarding the solicitation appears in the solicitation on section E.2 Addendum to FAR 52.212-1 Instructions to offerors - Commercial items.
