Description
JUSTIFICATION FOR SINGLE SOURCE AWARDS IAW FAR 13.106-1 (OVER MICRO-PURCHASE THRESHOLD($3K) BUT NOT EXCEEDING THE SAT ($150K)) IAW FAR13.104, COs must promote competition to the maximum extent practicable to obtain supplies and services from the source whose offer is the most advantageous to the Government, considering the administrative cost of the purchase. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source for purchases not exceeding the simplified acquisition threshold. COs may solicit from one source if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). IAW FAR13.106-3(b)(3),COs are required to include additional statements explaining the absence of competition (see 13.106-1 for brand name purchases) if only one source is solicited and the acquisition does not exceed the simplified acquisition threshold (does not apply to an acquisition of utility services available from only one source) or supporting the award decision if other than price-related factors were considered in selecting the supplier. This template when completed can be used to document single source awards IAW FAR13.106-3(b)(3). Note: Statements such as "only known source" or "only source which can meet the required delivery date" are inadequate to support a sole source purchase. 1. PURCHASE REQUEST OR REQUISITION NUMBER: 764-15-3-072-00031A. PROJECT/TASK NUMBER 1B. ESTIMATED AMOUNT: 70,000.00 2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: One Touch Ultra Glucose Test Strips. To be used with the One Touch Ultra Blood Glucose monitor for testing blood sugar levels. LANCET,PENLET PLUS DEVICE PRODIGY NO CODE (GLUCOSE) TEST STRIP SET,INFUSION MINIMED #MMT-377 RESERVOIR,MINIMED #MMT-326A RESERVOIR,MINIMED #MMT-332A CATH, EXTERNAL EXTENDED WEAR MED H#9207 Must be the original product from an authorized manufacturer and distributor. The items are required replenish our stock for filling prescriptions of veterans serviced by the (CMOP) Consolidated Mail Order Pharmacy. 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: Currently there is only one manufacturer for the seven (7) line items listed above. Monitor specific product. Do not have access to view patient profile for physician notes as to why specific contract test strip not chosen for prescription. Physician writes based on clinical need and to provide critical patient care. Manufacturer's warnings: Use only the reservoir and infusion sets specifically designed for your pump. Uses of non-Medtronic Diabetes reservoirs and/or infusion sets may reduce pump accuracy and hinder occlusion detection. Do not modify your reservoir or infusion set. VA doctors write prescriptions based on the clinical needs and the CMOP can only dispense what is written if there is a "do not substitute" order beside the prescription. 4. REASON THAT SUGGESTED SOURCE IS THE ONLY SOURCE, WHICH CAN PROVIDE THE SUPPLIES OR SERVICES: No generic substitution currently manufactured. Patients cannot use alternate type of test strip. Product is specific to the type of monitor the patient has. Other glucose test strips available on contract would not work in a monitor designed for the One Touch Ultra Test strips, likewise with the Prodigy No Code (Glusose) test strip. Lancets for the Penlet Plus Device are not interchangeable. In order to use an alternate product the patient would require a new lancet device. Use only the reservoir and infusion sets specifically designed for your pump. Uses of non-Medtronic Diabetes reservoirs and/or infusion sets may reduce pump accuracy and hinder occlusion detection. Do not modify your reservoir or infusion set. Therefore, brand name items are required to Fulfill the prescriptions written by the VA Pharmacist. 5. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: Market research was conducted and there are not items either on the NAC or GSA that satisfy this requirements for the seven (7) line items. The One Touch Ultra Glucose Test Strip is specifically required because patients have a blood glucose monitor where these strips are specifically designed for each meter. They are not interchangeable. Likewise for the Prodigy No Code (Glucose) Test Strips. These test strips are designed for a specific blood monitoring system and will not work in any other meter. The seven (7) line items are brand name, and will be competed open market, and small businesses will be able to quote on these items.
