- RFQ Template Wage Determination 2005-2064, Rev 17, 2015-07-08 Performance Work Statement for Weed Abatement on Vandenberg AFB CA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This combined synopsis/solicitation is issued as a Request For Quote (RFQ) PR# F4D3D15118A001. Submit written offers IAW CLIN structure outlined in announcement. Quote template sheet will be provided upon request. Oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM) (https://www.sam.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83 (effective 2 Jul 2015) and DFARS DPN 20150626 (effective 26 Jun 2015). North American Industrial Classification Standard (NAICS) 561730 and Size Standard of $7.5 million dollars apply to this procurement. Purchase Request #: F4D3D25118A001 Project Title : Weed Abatement for Main Cantonement of Vandenberg AFB 1. Quotes should conform to the CLIN structure as established in the RFQ Template, Attachment 1. 2. Quotes shall be valid through 30 Sep 2015. 3. Please see attached documents for detailed description of requirements. 4. All questions or comments must be provided to the Contract Specialist (CS) in writing via email NLT 03 Aug 2015. Answers will be posted NLT 5 August 2015. Telephone and other means of oral communication will not be permitted. 5. Quotes must be sent to Contract Specialist Mary Avants, mary.avants@us.af.mil No Later than 10 August 2015, 1200 PM (PST). The contract will be Firm Fixed Price with the CLIN structure reflected below. The period of performance will be for two (2) months - dates to be determined at time of award.Description of Services: Non-personal Services: The contractor shall provide all management, personnel, licenses, tools, supplies, equipment, materials, transportation, and labor necessary to provide post emergent herbicide treatments for abatement of broadleaf weeds and grasses on Vandenberg AFB, CA IAW the PWS. Areas to be treated with herbicides will include cracks at curbs, gutters, sidewalks, islands, pavers, rock beds, parking lots, and roadsides. Contractor shall use a marker dye to improve precision of application to reduce the need for re-performance of potentially missed areas. Maps will be provided to the contractor to illustrate the areas to be treated with herbicides.
