MTC Terms & Conditions Vendor Self-Certification Form W-9 Exhibit A Bid Sheet Request For Quote Hello Everyone, Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center, is requesting proposals for various dining service ware items. Details of the solicitation are in the attached request for quote (RFQ#TPJCC Dining Service Ware 15-15.pdf). All bids must be completed on the attached Exhibit A Bid sheet to be considered valid.
