- ATTACHMENT B - LIST OF PROVISIONS AND CLAUSES APPLICABLE TO FEMA RFP NO. HSFE50-15-R-0041 ATTACHMENT A - FEMA RFP HSFE50-15-R-0041 PRICING SPREADSHEET This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is being issued as Request for Proposal (RFP) No. HSFE50-15-R-0041 for bulk sodium chloride (road salt). The RFP clauses and provisions are those in effect thru Federal Acquisition Circular 2005-83, effective 07/02/2015. This acquisition is being solicited as a total service-disabled veteran-owned small business set aside. The NAICS codes applicable to this requirement are 325998 and/or 424690; small business size standards associated with these NAICS codes are 500 and/or 100 employees, respectively. Only one award will be issued as a result of this solicitation via the issuance of a simplified acquisition/commercial item purchase order. Please include all applicable Government discounts in your offer. The period of performance will be established as a base year with four one-year option periods. All deliveries shall be made to DHS, Federal Emergency Management Agency, Mount Weather Emergency Operations Center (MWEOC), 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135-2006. The following line items are required: Item No. 0001, Up to 800 tons of Sodium Chloride (bulk road salt) to be delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck during the base period of performance, September 1, 2015 through August 31, 2016 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton); Item No. 0002, Up to 800 tons of Sodium Chloride (bulk road salt) to be delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck during the Option Year One period of performance, September 1, 2016 through August 31, 2017 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton); Item No. 0003, Up to 800 tons of Sodium Chloride (bulk road salt) to be delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck during the Option Year Two period of performance, September 1, 2017 through August 31, 2018 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton); Item No. 0004, Up to 800 tons of Sodium Chloride (bulk road salt) to be delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck during the Option Year Three period of performance, September 1, 2018 through August 31, 2019 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton); Item No. 0005, Up to 800 tons of Sodium Chloride (bulk road salt) to be delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck during the Option Year Four period of performance, September 1, 2019 through August 31, 2020 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton). FOB Destination pricing is preferred; however if shipping/freight charges apply, please include the estimated shipping/freight rate for the base and each option year. Please reference ATTACHMENT A titled "FEMA RFP HSFE50-15-R-0041 Pricing Spreadsheet" for submitting price proposals. All applicable Government discounts should be included in your offer. FEMA is a tax exempt agency; please do not include sales tax on your quote. Source selection will be based on lowest cost, technically acceptable determination. In order to be considered technically acceptable, the contractor must provide evidence that it is a service-disabled veteran-owned small business and is registered in the System for Award Management (SAM); demonstrate the ability to deliver the required product on a "will call" basis (upon FEMA's request); and, demonstrate the ability to deliver Sodium Chloride (bulk road salt) in the quantities requested. FAR Provisions 52.212-1, 52.212-2, 52.212-3, 52.212-4 and 52.212-5 apply to this acquisition. Offerors are to include a completed copy of FAR provision 52.212-3 "Offeror Representations and Certifications--Commercial Items" with their response or provide this information via the System for Award Management (SAM) website, https://www.sam.gov/portal/public/SAM/. The successful offeror must provide their company DUNS number which is registered and active in the System for Award Management (SAM). A complete list of provisions and clauses applicable to this RFP is provided as ATTACHMENT B to this combined synopsis/solicitation. Contractor and Sub-Contractor personnel must comply with MWEOC security requirements. Contractors are required to become familiar with CFR 44, Part 15 and ensure any sub-contractors are also familiar with these rules and regulations before entering the MWEOC facility. QUESTIONS - Questions regarding this RFP are to be submitted in MS Word format to Phyllis.Woodward@fema.dhs.gov by 12:00 PM Eastern on August 6, 2015. Hand deliveries and/or faxes of questions will not be accepted without prior approval. Additionally, the subject line of your email shall, at a minimum, contain the RFP number. PROPOSAL SUBMISSION INSTRUCTIONS - The Contractor shall submit an electronic proposal via email to Phyllis.Woodward@fema.dhs.gov no later than 12:00 PM Eastern on August 17, 2015. The subject line of emails pertaining to this RFP shall clearly reference RFP # HSFE50-15-R-0041. Total size of email shall not exceed 20MB due to DHS email restrictions. Hand deliveries and/or faxes of proposals will not be accepted without prior approval. Contractor proposals shall be valid for 30 calendar days after the due date for responses to this RFP. A written notice of award or acceptance of proposal, emailed or otherwise furnished to the successful offeror shall result in a binding contract without further action by either party. Amendments to this RFP will be published in the same manner as the initial combined synopsis/solicitation.
