COMBINED SYNOPSIS / SOLICITATION DATE: 3 AUGUST 2015 PRODUCT: MEDICAL LABORATORY REPAIR SERVICES FOR ENDOSCOPES FACILITY: Augusta VA Medical Center, Augusta, GA AGENCY: Department of Veterans Affairs SET-ASIDE: SERVICE DISABLED VETERAN OWNED SMALL BUSINESS (VIP) This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. Interested parties should contact the Contract Specialist, Joseph Locke, joseph.locke@va.gov for a copy of the solicitation so that interested offeror(s) may complete the SF1449 document. Issue Date: August 3, 2015 Closing Date: August 7, 2015, 5pm EST Only e-mailed requests received directly from the offeror are acceptable. A brief description of the requirement is as follows: STATEMENT of WORK (1)PURPOSE: Repair of 2 Olympus Endoscopes to include: ( Cysto-Nephro Videoscope Model CYF-VH EE69318 Serial# 2301059 ) and ( Bronchoscope Model BF-1T180 EE55480 Serial# 2802356 ). (2)SCOPE of WORK: Serial# 2301059 requires major Olympus repair of the issue causing the scope to fail leak testing and replacement of the CCD video chip causing no video. Serial# 2802356 requires full factory refurbishment to repair the articulation issue, including the replacement of the CCD video chip causing no video. Both scopes will be returned to full factory specs. (3)PERIOD and PLACE of PERFORMANCE: Olympus Endoscopy Service Group 1350 Northmeadow Pkwy. Roswell, GA 30076 (4)BACKGROUND: Serial# 2301059 was sent to Olympus Endo Svc. Group for "Fails leak test". Serial# 2802356 was sent to Olympus Endo Svc. Group for "Not articulating properly". (5)PERFORMANCE OBJECTIVES: Return both scopes to OEM(Olympus) specifications. (6)OPERATING CONSTRAINTS: Repair is to be completed as soon as possible to prevent delays in patient care.
