- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W9124C-15-T-2069 and is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-83. The associated North American Industrial Classification System (NAICS) code for this procurement is 337215, Showcase, Partition, Shelving and Locker Manufacturing with a small business size standard of 500 employees. This requirement is a 100% small business set-aside. Offeror shall ensure their Representation and Certification in System for Award Management, and with the Small Business Administration self-certification under the associated North American Industrial Classification System (NAICS) code 337215, is identified as a small business entity. Performance Location: Third United States Army Central (USARCENT), Shaw AFB, SC 29152 REQUIREMENT DESCRIPTION: Line Item Description QTY 0001 Reconfigure 8' x 8' Haworth Workstations 60 EA 0002 Relocate all Removed Unused Components to Storage Warehouse 1 Job 0003 Relocation Electrical Spider Box 1 Job 0004 Install New Electrical Spider Box 1 Job 0005 Dual Monitor Arms 60EA 0006 Surge Protector 1EA This requirement includes: procurement of components for the reconfiguration of Haworth Workstations, delivery, reconfiguration of existing workstations, placement of all unused workstation components in a designated storage warehouse, relocation of Electrical Spider Boxes as required, procurement of surge protectors to accommodate two (2) personnel as required per workstation, and dual monitor arms. All new components color and pattern shall match the existing Haworth Workstations. RESPONSIBLE SOURCES SHALL PROVIDE THE FOLLOWING: 1. Price quote which identifies a complete breakout of requested item(s) unit and total price. 2. Point of contact to include: Representative name, phone and fax number, and email address. 3. Remittance address, Tax Identification Number, DUNS number and Cage Code number, and information pertaining to length of the base warranty. 4. Warranty information. 5. An itemized listing of components required for the reconfiguration for line item 0001. INSPECTION AND ACCEPTANCE: Destination by Government OFFER ACCEPTANCE: Quote MUST be valid for 60 calendar days after submission.
