Description
Project Vicinity Map PROJECT DESCRIPTION AND LOCATION Description of Work - Work shall be performed on roads #3130, #31302 and Welch Fire Trail Road. Work shall consist of road reconditioning, furnishing and placing Erionite Free aggregate, furnishing and installing a 16' cattleguard with concrete bases, end wings, object markers and bypass gate, and furnishing and installing a new pipe gate and other related road work. Work also shall be to furnish and stockpile 4 sets of concrete bases and four 16' full width cattleguard grids with end wings, and object markers. Contractor shall remove the existing gate and other related items. All removed items shall become the property of the Contractor and shall be disposed of in a lawful manner off of National Forest System Lands. UPDATE 8/6/15: All three roads combined are approximately six (6) miles in length. Note: When using the Forest Service concrete form for cattleguard bases. If ordering the grid and wings from Cow Country Equipment in Rapid City, it is recommended to order the "Montana" style grid and wings or the contractor shall have to customize the wings to fit within the grid and form. It shall be the Contractor's responsibility to customize and fit the wings to the grid if any other cattleguard manufacturer is used. The cattleguard installation is located on Road #3130 at the Forest Boundary. The location will be staked by the Government. All other cattleguards shall be stockpiled at the office trailer pad located off of road #3120 or other location approved by the COR. The gate installation is located on Road #3130 and will replace the existing damaged gate near the intersection with Road #3120. The location will be staked by the Government. Project Location - The project is located on the Sioux District, Custer Gallatin National Forest. The Site is approximately 100 miles north of Belle Fourche, South Dakota, 25 miles north of Buffalo, South Dakota, and 30 miles south of Bowman, North Dakota. The Site can be accessed by turning west off of U.S. Highway 85 onto the Tufte Road, 2 miles north of Ludlow; then turning left on the Riley Pass Road which turns into Forest Service Road (FSR) #3120 at the National Forest boundary. The main access roads within the Forest boundary are Forest Service Roads #3120 and #3130. OSHA Certification - The Contractor shall provide written documentation that the onsite project manager and all other employees engaged in the work has received the Occupational Safety and Health Administration 40 hour Hazardous Waste Operations and Emergency Response (HAZWOPER) Training required under 29 CFR 1910.120. The Contractor must provide copies of certifications, including annual refresher training certifications, prior to issuance of notice to proceed which is estimated to be issued 2-3weeks after award. The Contractor shall utilize only onsite personnel that have completed HAZWOPER training when work is being performed. Vendors delivering supplies to the site are not required to complete HAZWOPER training. Completed HAZWOPER training is not a condition of award but shall be completed prior to issuance of notice to proceed, thus this will need to be coordinated and scheduled in a manner such that it will not impede progress on the awarded contract. If HAZWOPR training is not already complete at time of bidding, evidence of scheduled training shall be submitted with the contractor's bid package indicating completion by the estimated notice to proceed issuance time frame. Erionite - The Tufte Road, a portion of road #3120, and other roads in the project area may contain Erionite, a zeolite mineral that, in its fibrous form, may have properties similar to asbestos. Erionite may pose health risks to those who breathe in the fibers. The Contractor shall take measures to mitigate the risk of airborne exposure from sediment and Erionite. All aggregate imported to the site shall be tested by the Contractor and shall be non-detect for Erionite by the Polar Light Microscopy (PLM) method. Contractor shall submit test results showing pit location, name, and lab test results before any aggregate is imported to the site. UPDATE 8/6/15: Two known and tested sources of erionite free material are: Fisher Sand & Gravel of Spearfish, SD and Hills Materials of Rapid City, SD. Other sources may exist and it is incumbent upon the Contractor to locate and have the materials tested ensuring "non-detect" for erionite content. Price Range - Between $500,000 and $1 Million per FAR 36.204(e) Pre-Bid Tour - A pre-bid tour will be held for those interested in viewing the project. The majority of road #3130 and #31302 are behind a locked gate. Bidders not attending the tour will not be granted driving access and will have to walk in order to view the roads. All bidders are encouraged to visit the project site. For additional contract information contact David Shimek, COR, at the Billings Office Custer Gallatin National Forest at (406)255-1422 for contract information. The specific Pre-Bid Tour date/time will be posted on FBO in sufficient time to allow prospective bidders time to plan. UPDATE 8/6/15: A pre-bid tour will take place on 8/27/15. Interested Contractors shall contact the project engineer, David Shimek at (406) 255-1422 no later than 4:30PM Mountain on 8/26/15 to register for the pre-bid and to obtain the meeting time and location. If there are no registrants, the pre-bid site visit will be cancelled. Start Work - UPDATE 8/6/15: It is estimated that the work will begin October, 2015. Performance Period - UPDATE 8/6/15: All work shall be complete by 15 June 2016 given notice to proceed by 15 October 2015. If notice to proceed is not issued by 15 October 2015, the period of performance shall be extended by the number of days past 15 October 2015 the notice to proceed is actually issued. The contract time allows for manufacture and delivery of materials, weather delays and winter exclusion for completion of all work items. Upon starting work at the cattleguard, the Contractor shall prosecute the work in a diligent manner so as to complete the project within the period of performance. Government-Furnished Materials - If available, the contractor may use the Forest Service owned concrete form for cattleguard concrete bases, which is located at Fallon County Ready Mix in Baker, Montana. It shall be the contractor's responsibility to have the concrete bases constructed, loaded, delivered, and unloaded from Fallon County Ready Mix to the installation and stockpile sites. It shall be the Contractor's responsibility to coordinate all work and supplies related to the bases. Work Schedule - The Contractor shall supply a work schedule before starting the project. The Contractor shall notify to Contracting Officer a minimum of 48 hours in advance of changes in the work schedule. Use of Roads by the Contractor - The Contractor is authorized to use roads under the jurisdiction of the Forest Service for all activities necessary to complete this contract, subject to the limitations and authorizations designated in Road Order(s) or described in the contract, when such use will not damage the roads or National Forest System resources, and when traffic can be accommodated safely. Traffic Control - Existing roads shall remain open during the project. The Contractor may close the road for short periods of time, however the closure shall not exceed 60 minutes while vehicles are waiting. Access to the cattleguard bypass gates shall be constructed and maintained for traffic prior to starting work on the cattleguard. All road and traffic related signing shall be in accordance with the Manual on Uniform Traffic Control Devices (MUTCD). Perform work in a manner that ensures the safety and convenience of the public and protects the residents and property adjacent to the project. Accommodate public traffic on roads adjacent to and within the project until the project is accepted. Camping - Camping is not permitted on the site. Cleanup and Disposal - The Contractor shall be responsible for solid waste disposal. The Contractor shall at all times keep the area in a clean, neat, and orderly condition. Construction related debris, including but not limited to used motor oil, oil cans, grease cartridges and refuse shall be removed promptly from the site and disposed of in a lawful manner off National Forest System lands. Fees for the use of landfills or transfer stations shall be paid for by the Contractor if there is a charge. Security and Protection - Security is the responsibility of the Contractor. The Contractor shall be responsible for protecting all equipment and supplies for use on the project and stored on the site. The Contractor shall be responsible for all costs required to provide such protection. Barricades, Warning Signs, and Lights - Comply with standards and code requirements for erection of structurally adequate barriers. Signing shall meet MUTCD Standards and advise the public of the work being done. Existing Utilities - The Contractor is responsible for location of all utilities prior to ground disturbance. Damaged utilities shall be repaired at the Contractors expense. Contact One Call prior to any ground disturbance. Safety Plan - Contractor shall provide adequate safety briefings and ensure safe practices are followed by all personnel involved in the project. Emergency contact numbers shall be readily available on site. A safety plan shall be submitted to the CO prior to starting work. Submittals - The contractor shall submit current 40 hour OSHA HAZWOPER Certifications for all employees working on the site. Other submittals include Aggregate Sampling Plan, Work Schedule, and Safety Plan. Submit to the CO one week prior to starting work. The Contractor shall regularly update the Work Schedule. Contractor shall submit all test results, testing samples, and other submittals for approval. Contractor shall submit aggregate testing results and shall provide Erionite non-detect test results prior to furnishing aggregate to the project. Disturbances - The Contractor may work Monday through Friday excluding legal holidays unless approved in writing by the CO. Staging Area - Shall be approved by the CO. Protection of Improvements - The Contractor shall complete sequences of construction activity in order to prevent damage to equipment, material, and articles incorporated into the work covered by this contract. Water - There is no potable water available on this job. Contractor shall be responsible for all water including water for construction. Toilet Facilities - The Contractor shall supply portable toilets to the project site. Specifications - This contract utilizes FP-03 Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects. The Contractor shall have a copy of the specifications on site and shall be responsible for providing their own copies. Bonding Requirements - A Bid Bond - SF-24 in the amount of 20% of the bid price or $200,000 (whichever is less) will be required to be submitted with the Contractor's bid. The awarded Contractor will be required to submit Payment and Performance bonds equal to 100% of the bid price (each bond) within 10 days after award of the contract. Solicitation - This project will be advertised as a 100% Total Small Business Set-Aside under NAICS code 237310. It is anticipated the full solicitation and all bidding materials will be posted to the FBO website on/around August 19, 2015.
