- NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Request for Quotation (RFQ) is VA786-15-Q0373. No other solicitation will be issued in response to this announcement. This announcement will serve as both synopsis and the solicitation. Quotes in response to this combined synopsis/solicitation are due August 27, 2015 @ 2:00PM MT and shall be emailed to Geraldine.herrera@va.gov. Purchase order award will be made to the lowest priced quoter and only one award will be made from this solicitation. This RFQ document incorporates provisions and clauses which are in effect through Federal Acquisition Circular FAC 2005-83. The type of purchase order contemplated is firm fixed price and the actual number of trees will be determined at time of award based upon funding available. The associated North American Industrial Classification System (NAICS) code for this procurement is 561730 with a small business size standard of $7.0M. This requirement is set-aside 100% for Service Disabled Veteran Owned (SDVOSB) Small Business Concerns and only qualified and eligible SDVOSB concerns may be considered for possible purchase order award. The period of performance is 30 calendar days from receipt of purchase order award. The place of contract performance is Houston National Cemetery located at 10410 Veterans Memorial Drive, Houston, TX, 77038. Interested parties may contact the point-of-contact Ms. Geri Herrera at 303-914-5703 or the cemetery-point-of-contact @ 281-447-8686 to schedule a site visit. The contractor shall provide both a unit price and total amount for all Contract Line Item Numbers (CLINS), and all pricing shall be based on the attached statement of work. The contractor shall provide all plant, labor, tools, parts, equipment, transportation and incidentals needed to perform work as shown on the attached Bid Schedule. Quoters must be registered in the System for Award Management (SAM) at https://www.sam.gov to be eligible to receive award. In addition, SDVOSB concerns must be registered and have an active account in www.vetbiz.gov.
