- ATTACHMENT A-3A RFQ DTPH5615Q00010 ***THIS IS A TOTAL SMALL BUSINESS SET ASIDE*** The U.S. Department of Transportation (DOT), Pipeline and Hazardous Materials Safety Administration (PHMSA), Office of Pipeline Safety (OPS) has a requirement for procuring uniform shirts and windbreaker jackets for the DOT PHMSA nationwide. You are invited to submit a quote in response to PHMSA's Request for Quotation (RFQ)# DTPH5615Q00010. This requirement is being solicited under FAR Part 12 & 13. Please see the attached Statement of Work (SOW) for the description of the supplies to be purchased under this RFQ. Also, please read this document in its entirety as it contains pertinent information (quote contents, evaluation factors, due dates, etc.) regarding this RFQ. All questions regarding this RFQ must be emailed to Shaukat Mirza ( Shaukat.Mirza@dot.gov ) with a copy to Mr. Warren Osterberg ( Warren.Osterberg@dot.gov ). -Do not contact any other PHMSA personnel during the solicitation phase. All questions must be received on or before Friday, August 21st, 2015 at 1:00 pm, EASTERN TIME. DEADLINE OF QUOTATIONS: Please send quotes via email to Mr. Shaukat Mirza ( Shaukat.Mirza@dot.gov ) with a copy to Mr. Warren Osterberg ( Warren.Osterberg@dot.gov ). Written Quotes must be received on or before 6:00 PM EASTERN TIME on Tuesday, September 08th, 2015.
