The United States Property and Fiscal Office of the Oregon Army National Guard is soliciting quotes to procure the following supply or services: CLIN 0001: Provide Physical Health Assessments in accordance with Part 5 of the Performance Work Statement and Exhibit 1. CLIN 0002: Provide Physical Health Assessments in accordance with Part 5 of the Performance Work Statement and Exhibit 2. CLIN 0003: Provide Dental Exams, Dental Treatment and Hearing Screenings in accordance with Part 5 of the Performance Work Statement and Exhibit 3. CLIN 0004: Provide Physical Health Assessments in accordance with Part 5 of the Performance Work Statement and Exhibit 4. CLIN 0005: Provide Dental Exams, Dental Treatment and Hearing Screenings in accordance Part 5 of the Performance Work Statement and Exhibit 5.
