Description

The Department of Veterans Affairs (VA) and the VA Central Iowa Health Care System, Des Moines (VACIH Des Moines) plans to issue a Request for Proposal to obtain Pathology services to eligible beneficiaries. The Contractor shall provide Board Certified Pathology services on site, AND associated pathology reference testing and consultative services (off-site) in accordance with the specifications contained in the Performance Work Statement in the attached RFP VA263-15-R-0588. This is a pre-solicitation notice for commercial services prepared in accordance with FAR Parts 12 Commercial Item and 15 Negotiated Contracts. This solicitation will be an unrestricted acquisition, small business will be preferred. The solicitation will be issued on or about August 10, 2015 with an anticipated proposal due date of September 8, 2015 via Federal Business Opportunities: https://www.fbo.gov/ The solicitation contains requirements for a 12-month base year with two (2) twelve-month options to renew. Service will begin December 1, 2015 (or date of award) through November 30, 2016 for the Base Year. The Offeror is responsible for downloading all documents including amendments without further notification from the VA Contracting Officer. The RFP must be completed in its entirety and included in the response. Proposals must be submitted in writing to the issuing office, be in accordance with the preparation instructions outlined in the solicitation, and be timely. Late proposals will not be accepted. The North American Industry Classification System (NAICS) code assigned to this procurement is 621111 "Offices of Physicians, except Mental Health Specialists," with a size standard of $11.0 million. Offerors will be evaluated on technical ability, past performance, socio-economic status and price. Contractors must be registered in the System for Award Management (SAM). Lack of registration in the System Award Management (SAM) database (www.sam.gov) will make a vendor ineligible for award. https://www.sam.gov/ This solicitation incorporates provisions and clauses in effect through FAR and VAAR. It is the contractors' responsibility to be familiar with the applicable clauses and provisions.
