- RFQ HSCEDM-15-Q-00014 Statement of Work and equipment specifications. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written (formal) solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ) solicitation number HSCEDM-15-Q-00014 in accordance with FAR Parts 12 & 13. Please read and comply with all requirements for submitting a quote. Failure to comply with all instructions contained within this combined synopsis/solicitation will result in the quote being ineligible for award. It is the offeror's responsibility to ensure their quote meets all the requirements identified herein. The Department of Homeland Security (DHS), Immigration and Customs Enforcement (ICE) is requesting a quotation to provide the Boise Sub-office, located at 620 East 10th Avenue, Suite 201, Anchorage, AK 99501, gym equipment for ICE officers fitness requirement, contractors must be able to meet the following Government requirements: 1) Contractor must provide Pictures and Specifications for all suggested equipment. Provide, install and warranty the following equipment: • Quantity: 1 o Smith Cage System with Dual weight stack design, double racking system to accommodate smith bar and free weight bar simultaneously, adjustable high/low pulley system, with Pull-up bar -adjustable in height, with FID bench as well as preacher curl and leg attachments, bearing System, two 200lb weight stack commercial weight stacks with solid guide rods and center drilled weight plates. o The following accessories must be included  Foot-bench,  Leg Ext Pulley kit  Two pair of "D" handles one long, one short and pull-up assist strap. o Due to limited space in the gym area, this equipment's dimensions are limited to a maximum height of 88", 76" wide and 76" in length. • Quantity: 1 o Stationary Bike - Upright, cordless with rechargeable batteries, four-way adjustable seat, with electronic braking and programing, customized workout programs. o Due to limited space in the gym area, this equipment's dimensions are limited to a maximum height of 88", 27" wide and 46" length. • Quantity: 1 o 55-75lb rubber coated hex dumbbells in 5 lb. increments for a total of 650 lbs., with ergonomic grip design. o 3-tier dumbbell rack; must be capable of storing 10 pairs of Hex dumbbells and at least 1160 lbs. Due to limited space in the gym area, this equipment's dimensions are limited to a maximum height of 88", 21" wide and 55" length. 2) Contractor shall deliver all equipment specified on the above list and install all equipment in the workout room. There is a loading dock and the workout room is on the ground level. All quotes must include pricing for FOB- destination. 3) Contractor shall contact Oscar Durazo at (907)-271-3486 10 days after contract award to schedule date and time of delivery of equipment. 4) Contractor shall provide warranty work with the submitted quote. Period of performance will begin within 10 days of award and continue through date of installation and any warranty periods. The Government will award on a lowest price technically acceptable basis. The following FAR Clauses are incorporated by reference, the Offeror may obtain the full text of the clauses electronically at web site http://www.arnet.gov/far: FAR 52.212-2 Evaluation-Commercial Items applies with evaluation factor of: (1) Lowest price technically acceptable. FAR 52.212-1 Instructions to Offerors-Commercial Items; FAR 52.212-3 Offeror Representations and Certifications-Commercial Items and submit their representation and certifications FAR 52.212-4 Contract Terms and Conditions-Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items Request for Quotation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-81. The North American Industry Classification System Code (NAICS) is 339920: size standard is 500 employees. This requirement is small business set aside. Closing date or response date for this synopsis/solicitation is August 13, 2015 at 8:00am, Pacific Time. Only e-mail or fax responses will be accepted.
