- This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is W912JV-15-T-4005 and it is issued as a Request for Quotation (RFQ). All References to quote mark offer quote mark or quote mark offeror quote mark in the clauses and provisions incorporated below are deemed to have the meaning of quote mark quotation quote mark and quote mark quoter quote mark respectively. Any quotation received in response to this RFQ is not binding until the contractor either signs the purchase order issued at time of award, or starts performance. This RFQ and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-76 and Defense Acquisition Circular (DAC) 91-13 (Correction). This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (b). The NAICS code is 621399 and the small business size standard is $7,500,000. This requirement will seek services under a base plus four option year contract under FAR 17.204(e). The following commercial items (services) are requested in this solicitation; Physical Therapist (AA) QTY: Not to exceed (NTE) 2,080 hours for base and each option year (if exercised). Base year Period of Performance (PoP): 9/30/15 - 9/29/16, option year PoP 9/30/xx - 9/29/xx Unit of Issue: Hours - see section 9 of the attached Performance Work Statement (PWS) Athletic Trainer (AB) QTY: Not to exceed (NTE) 2,080 hours for base and each option year (if exercised) Base year Period of Performance (PoP): 9/30/15 - 9/29/16, option year PoP 9/30/xx - 9/29/xx Unit of Issue: Hours - see section 9 of the attached Performance Work Statement (PWS) Strength & Conditioning Coach (AC) QTY: Not to exceed (NTE) 2,080 hours for base and each option year (if exercised) Base year Period of Performance (PoP): 9/30/15 - 9/29/16, option year PoP 9/30/xx - 9/29/xx Unit of Issue: Hours - see section 9 of the attached Performance Work Statement (PWS) Questions and Answers: Should you have any questions about this requirement, please submit them no later than 4:00 PM EST 20 August 2015 to steven.s.dong.mil@mail.mil. All questions must be submitted via email. Answers to all questions will be posted to FBO no later than COB 27 Aug 2015. All quoters must be System for Award Management (SAM) registered and the online representation and certification in SAM must be completed. Information concerning SAM requirements may be viewed via the internet at https://www.sam.gov or by calling 866-606-8220, or 334-206-7828 for international calls. Quoters that reject the terms and conditions of the solicitation may be excluded from consideration. The following provisions and clauses apply to this acquisition: FAR 52.212-1,Instructions to Offerors--Commercial Items, FAR 52.212-2 Evaluation --Commercial Items and award will be made to the quoter whose quotation is judged to represent the best value to the Government, on the basis of price, past performance and technical factors for CLIN 0001 (base year), CLIN 0002 (option year 1), CLIN 0003(option year 2), CLIN 0004 (option year 3) and CLIN 0005 (option year 4), FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items. The provision at FAR 52.212-1, Instruction to Offerors - Commercial Items, applies to this acquisition: - Quoted prices must be submitted using the Pricing Sheet within attachments. - Quoters shall insert unit prices in the attached Pricing Schedule, rounded to the nearest cent and sign acknowledging the terms and conditions of the RFQ. Quoters rejecting the terms and conditions may be excluded from consideration. Note: Department of Labor (DOL) Wage Determination 2005- 2441 Rev. 16, dated 8 July 15, at Attachment 2, applies. - A maximum of five (5) references will be accepted for past performance evaluation. References for past performance shall include a point of contact, their phone number, email address, name of the company, and detailed description of services rendered. Other information should include the contract number, initial contract cost, and period of performance. - Management Plans must be submitted for technical evaluation. * Staffing, quality control, capabilities, and experience (not to exceed 3 pages, 10 point font) * Organizational Chart (not to exceed 1 page, 10 point font) * Scheduling Plan (not to exceed 1 page, 10 point font) * Substantiating documents to include, but not limited to, resume(s), diploma(s), and certification(s) to demonstrate qualified personnel possess all of the position requirements per the attached PWS - section 10. - All quotes must be sent to MSgt Steven Dong via e-mail at steven.s.dong.mil@mail.mil. Quotes are required to be received no later than 12 September 2015, 10:00 AM PST. EVALUATION FACTORS Evaluation: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order to the quoter whose quotation is judged to represent the best value to the Government, on the basis of price, past performance and technical factors. All factors other than Cost or Price combined (Past performance and Technical factors) are significantly MORE IMPORTANT than Cost or Price. Cost or Price: Evaluation of price will consider the total of the annual prices (CLINS) for all contract years (base plus all options, including a possible six-month extension), with each year's price (CLIN) evaluated as the sum of the prices of all subcontract line items (SubCLINs) for that year. Each SubCLIN price will be evaluated as the quantity (hours) multiplied by the unit price, rounded to the nearest cent. A potential six-month extension IAW the clause FAR 52.217-8 will be evaluated at the pricing of the final option year. In the event of an extension under this clause, it will be exercised at the prices of the prior contract year. Past Performance: Evaluation of past performance will consider references provided and past performance information obtained from any suitable source, such as ratings from automated past performance data bases (i.e. CPARS, PPIRS), direct local business experience with the quoter, or similar. Past performance will be rated as Acceptable or Unacceptable. Technical Factors: Evaluation of technical factors will consider subfactors in the following categories: * Capability and experience, * Organization/organizational structure, * Qualified personnel, * Quality control, and * Whether or not all of the Management Plans required above were submitted and deemed adequate. Quotes will be evaluated as outstanding, good, acceptable, marginal, and unacceptable for Technical Factors.
