- Base Access Affidavit Base Access Request Memo template SOW, F1T0CC5153A001 Giant Voice Duke Field SOW SF 1449, FA2823-15-T-3002 Giant Voice Duke Field The Air Force Test Center, Installation Operational Contracting Division, Eglin AFB, Florida intends to solicit and award a firm-fixed price contract for a contractor to provide all labor, equipment, materials, transportation and any other items or services necessary to remove the existing outdoor Mass Notification System (MNS), also known as the Giant Voice (GV); engineer, design, and install a new outdoor MNS, and provide hands-on training to the 919 SOW/CP, Duke Field, FL in accordance with the attached Statement of Work (SOW). This is a combined synopsis/solicitation for commercial items prepared under Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures, and the procedures in Subpart 13.5 Test Program for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is Request for Quotation numbered FA2823-15-T-3002. The North American Industry Classification System (NAICS) code for this acquisition is 334290 with a size standard of 750 employees. This is a 100% total small business set aside; please identify your business size in your response based upon this standard. In accordance with the FAR 13.106-1(a)(2) offerors are notified that award will be made to the lowest priced offeror that meets the minimum requirement or presents the best value to the government. Offeror will be evaluated in accordance with FAR 13.106-2(b). Offerors must submit a quote for the item listed as well as complete the required documentation. Partial offers will not be considered for award. 1. Item: CLIN 0001 Description: Contractor shall provide all labor, equipment, materials, transportation and any other items or services necessary to remove the old MNS; procure, design and install a new MNS and provide hands-on training to the 919 SOW/CP, Duke Field, FL in accordance with the Statement of Work. Quantity: 1 Unit: Lot Price: 2. Required documentation: a) Complete the Blocks 12, 17a, 17b, 30 a-c on the Standard Form (SF) 1449. An authorized official of the firm must sign and date the SF 1449 in Block 30 a-c. b) Enter proposed UNIT PRICE, AMOUNT, and NET AMT on page 3 of the SF 1449. c) Complete 52.212-3 as applicable on pages 4 - 18 of the SF 1449. d) Complete 252.209-7998 (Dev) on page 25 of the SF 1449. e) Complete 252.209-7999 (Dev) on page 25 of the SF 1449. f) Complete 252.211-7003 as applicable on page 25 - 31. Offerors must provide, with their offer, a breakout of the individual components, equipment, materials and labor/installation to include manufacturer and P/N of the equipment being proposed. In accordance with, FAR 52.212-1(b)(4), offerors are reminded that as a minimum, offers must show a technical description of the items being offered insufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary. Quoted price should include FOB Destination and be valid at least through 30 September 2015. Required delivery date is no later than 180-days after receipt of order. Expedited delivery schedules accepted and encouraged. SITE VISIT: A site visit will be held on 24 August 2014 from 1000-1700 hours CST or (10am-5pm) CST. BASE PASSES FOR SITE VISIT: Attendees shall complete a Base Access Request Memorandum and a Base Access Affidavit (see attachments). The forms shall be completed and emailed to sherwyndon.ballesteros.1@us.af.mil and ronald.wilson.22@us.af.mil. The memorandum and affidavit will then be submitted to Security Forces by the Contract Specialist. Security Forces will perform a background check on all contractors visiting. When sending emails to the Contract Specialist please check your email options for ‘obtaining a delivery receipt' to allow verification of the email being delivered. The access requests for both forms should only be for Duke Field, Buildings 3077 and 3021 for 24 Aug 2015 from 1000-1700 hours CST or (10am-5pm) CST. * Ensure the memorandum and the affidavit are emailed/received by the Contract Specialist (at a minimum of) four (4) work days (M-F) before the day of the site visit so that Security Forces will have enough time to process the required forms and perform the required background check(s). Please be advised to not expect to get on base with a retired military or dependent ID card; Security Forces does not allow the use of these cards when doing business on base. All offeror/contractor personnel are required to notify the Contract Specialist by email (if planning on attending the site visit. QUESTIONS AND ANWERS: All questions must be emailed to sherwyndon.ballesteros.1@us.af.mil and ronald.wilson.22@us.af.mil no later than 3 calendar days after the site visit. Any questions submitted after this deadline may not be answered. All questions and subsequent answers will be posted to www.fbo.gov under this solicitation number. Offerors are responsible for monitoring www.fbo.gov for updates.
