- Request for Proposal Location Bamberg Job Corps Center 200 Carlisle St Bamberg, SC 29003 Building: Memorial Dormitory, Building 07 Site Contact: Mr. Joe Hajba Purchasing Coordinator (803) 245-6320 Project: Removal of existing ACM floor tile and mastic, installation of new VCT and cove base. Findings: The existing VCT flooring and cove base is worn or damaged Scope of Work: Removal of existing 12"x12" ACM containing floor tiles and mastic. The contractor will be responsible to obtain necessary permits as required. Removal of ACM containing floor tiles and mastic to be completed by a licensed asbestos contractor in accordance with State and Federal regulations related to asbestos removal. An independent air monitor shall provide air-sampling during the removal of the asbestos flooring and mastic. The air sampler to provide final clearance air-testing. The contractor shall dispose of all ACM materials in an EPA approved landfill. Installation of new VCT and 4" vinyl cove base. Tiles to be 12"x12" by 1/8" installed with low VOC tile adhesive. Undercut wood doors as required to provide required clearances. VCT shall be Armstrong, Mannington, Johnsonite or other approved manufacturers. Cove base shall be Johnsonite, Roppe or other approved manufacturers. The center will approve tile and cove colors based on samples provided by contractor. Reference: NESHAPS, 2014 40CFR Part 61, Subpart M and ASHA 2014, CFR 1910.1001 General Conditions: Contractor must be licensed and insured to provide this type of service. Davis-Bacon regulations are in affect. All work to be code compliant and installed in a safe and professional manner. Warranty or guarantee of work should be included in bid. Bid proposal to include breakout of material, labor and mark-up. No work may be started until a Purchase Order has been issued. All change requests must be approved by Purchasing Coordinator in advance. Bids should be good for 60 days unless noted. Contractor will be responsible to remove the existing flooring and cove base and dispose/recycle of properly. There should be no un-necessary interaction between contractor and the Westover Job Corps Center students. Where applicable, all equipment manuals and service documentation to be turned over to the facilities manager. Work to be scheduled in advance with the facility manager. All contractors working at the Bamberg Job Corps Center are required to sign in and out each day. Vehicles entering and exiting the center are subject to search. Contractors must dress appropriately and visitor's badges must be worn. Tools and material must be secured at the end of each day. Alutiiq Education and Training accepts no responsibility for tools and materials stored. All contractors are subject to the Bamberg Job Corps Center rules and regulations for work at this site. This center is non-smoking. Note: Project may be cancelled if funding withdrawn or not available
