Addendum to 52.212-2 Evaluation -- Commercial Items Addendum to 52.212-1 Instructions to Offerors -- Commercial Items FA4610-15-T-0006 Portable Toilets RFQ Portable Toilets Experience Questionnaire Portable Toilets Price Schedule WD 2005-2064 (Rev.-17) Portable Toilets PWS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for quote (RFQ) FA4610-15-T-0006. Submit written offers IAW CLIN structure outlined in announcement. Oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM) (https://www.sam.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83 and DFARS DPN 20150626, (effective 26 Jun 2015). This procurement is a 100% Small Business set-aside IAW FAR 19.502-2(b). North American Industrial Classification Standard (NAICS) 562991, Portable Toilet Renting and/or Servicing, and Size Standard of $7.5 million dollars apply to this procurement. Purchase Request #: F4D3D15177A001 Project Title: Rental/Servicing of Portable Toilets 1. Quotes should conform to the CLIN structure as established in the attached RFQ (FA4610-15-T-0006). 2. Quotes shall be valid through 31 October 2015. 3. Please see attached Performance Work Statement (PWS) for detailed description of requirements. 4. The resulting contract will be Firm-Fixed Price (FFP) and the CLIN structure is reflected in the attached RFQ (FA4610-15-T-0006). The period of performance will be for one (1) basic year, four (4) one (1) year option periods, and an option to extend for a period Not To Exceed six (6) months. Services, Non-Personal: The contractor shall provide all management, personnel, tools, supplies, equipment, materials, and labor necessary to deliver and place, maintain, and remove fully operational portable toilets at government designated sites within the boundaries of Vandenberg Air Force Base, California. Services shall be performed in accordance with (IAW): this Performance Work Statement (PWS); applicable commercial/industry standards; American National Standard Institute (ANSI) requirements (ANSI Z4.3, Section 5, Requirements for Portable Toilet Facilities and Portable Toilet Rooms); Occupational Safety and Health Administration (OSHA) and Environmental Protection Agency (EPA) requirements; manufacturers' recommendations; Americans with Disabilities Act (28 CFR, Part 35, Subpart A); and all applicable federal, state, and local laws/requirements. Contractor shall provide portable toilet rental and service to buildings/areas identified by work order.
