- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. JOINT BASE LEWIS-MCCHORD intends to award two BLANKET PURCHASE AGREEMENTS that are advantageous to the U.S. Government, on a firm-fixed-price contract for Mortuary Services. The associated North American Industrial Classification System (NAICS) code for this procurement is 812210 with a small business size standard of $7.5 million. This requirement is a total SMALL BUSINESS set-aside and only qualified offerors may submit quotes. JOINT BASE LEWIS-MCHORD requests quotes for a 3 year BLANKET PURCHASE AGREEMENT. Contractors must be located within a 50-mile radius of JBLM, WA, 98438. This solicitation is issued as a Request for Quotes using lowest priced- technically acceptable and offerors' qualifications to select. Offerors shall consider the information included in all solicitation attachments, and shall submit responses in accordance with these instructions. To be considered responsive offerors shall submit the following documents: completed Schedule of Service, General Price List and copy of state licenses. All CLINs shall be quoted FOB Destination to Joint Base Lewis McChord, WA 98438. Ffailure to provide these documents by the date and time set for responses may result in the quote being considered non-responsive and ineligible for further consideration. Offerors shall submit responses via email to both Alfonso Nolasco, Contract Specialist, and SFC Edwin Berry, Contract Officer, at the following addresses: alfonso.nolasco.civ@mail.mil and edwin.l.berry.mil@mail.mil no later than 12:00 noon Pacific Time on the due date set for responses. Questions regarding this solicitation shall be directed to the Contract Specialist and Contract Officer via the email addresses listed above, preferable no later than three (3) business days prior to the due date for responses. Questions received later may not provide sufficient time for adequate responses, and the government reserves the right to proceed to close the solicitation on schedule regardless.
