Description

The VA Coatesville Medical Center located at 1400 Blackhorse Hill Road, Coatesville PA. 19320 seeks to contract with a dental laboratory to provide partial and full denture services, as well as soft and hard mouth guards in support of our non-homeless veteran population. Services provided need to be of high quality, efficient, and cost effective. Completed and partial dentures, PFM crowns and bridges that are metal based, but lead-free, are required. It is estimated that these services would be required for approximately 200 to 250 denture cases and 200 to 250 crowns annually. For additional details on this service (including estimated quantities) please see the attached statement of work. This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (large business, small business, 8(a), SDVOSB, etc.), relative to NAICS code 339116 (size standard of 500 employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Based on responses to this synopsis, the Government may issue this RFQ for these services on eBuy or Fed Biz Ops. Estimated Performance Period: Base Period:October 1, 2015 through September 30, 2015 Type of Contract: Firm Fixed Price, Requirements This notice is not a request for competitive proposals, however; any responsible source who believes it is capable of meeting the requirement may submit a capability statement to the contracting office no later than Monday, August 24, 2015, at 3:00 PM, EST. In the subject line of the email, please include the following description: COATESVILLE DENTAL-VA244-15-Q-1295. No telephone responses will be accepted. This notice is to assist the Government in determining sources only. A solicitation is not currently available. If a solicitation is issued, all interested parties must respond to that solicitation announcement separately from the responses to this announcement. ? STATEMENT OF WORK -Non-Homeless Veterans Dental Lab OVERVIEW: The Coatesville VAMC seeks to contract with a dental laboratory to provide partial and full denture services, as well as soft and hard mouth guards in support of our non-homeless veteran population. Services provided need to be of high quality, efficient, and cost effective. Completed and partial dentures, PFM crowns and bridges that are metal based, but lead-free, are required. It is estimated that these services would be required for approximately 200 to 250 denture cases and 200 to 250 crowns annually. STATEMENT OF AWARD: The government intends to award a contract as a result of this solicitation. This solicitation is for local vendors only. A local laboratory is defined as one which is close enough in proximity to the Coatesville VAMC such that pick-up and delivery of casework by a contractor representative can be routinely provided, personal face-to-face discussions can be held on occasion as needed at the VA site, or if applicable at the contractor's place of business. STANDARDS OF PERFORMANCE: The contracted laboratory shall fabricate all prostheses and perform all laboratory steps to the highest standard of quality, with special attention given to neatness and attention to laboratory prescription detail. Contractor's laboratory facility shall be certified. This solicitation includes regulations under the Buy American Act; VA Medical Center requires that all dental appliances supplied to the VA dental clinics are manufactured in the United States using high quality materials that meet American National Standards Institute (ANSI) and Food and Drug Administration (FDA) regulations. Dental laboratories must also comply with the regulations governing FDA quality system/good manufacturing practices for products distributed in the United States dental laboratories. In addition, FDA prohibits the use of lead in the surface material used to fabricate dental prosthesis. At award, contractor will be required to sign a letter certifying compliance with regulations from VA Memorandum "Safeguard to Prevent Use of Unapproved Materials in VA Dental Appliances" dated April 17, 2000. The Government reserves the right to require "remake" at no additional cost if the casework does not conform to a high standard of quality, and the deficiency is clearly caused by the contractor. Chief, Dental Service, VAMC Coatesville will have final determination on high standard of quality, and the deficiency after consulting with the contractor. PICKUP & DELIVERY OF CASEWORK: Inside pick-up and delivery to the Coatesville VA Dental Service is expected. The inside point of pick-up/delivery will be specified upon award. The particular times for scheduled pickups will be determined by mutual agreement between the VA Dental Service Chief and the Contractor. Transportation of casework shall be provided at no additional cost to the VA; therefore, unit pricing of the Complete Denture Inclusive Service (including impression trays, record bases, setup, DENTSPLY Classic teeth, and processing), should appropriately reflect transportation costs. Successful awardee shall either provide pickup and delivery services by car (if local) or by the use of prepaid commercial delivery service. A Quality Assurance Surveillance Plan will be a part of this contract. Dr. Gary Lobasso, Chief Dental Service will determine the quality of the products provided.
