Description
Questions and Answers Amendment No. 1 : 20 August 2015 - The purpose of this amendment is to provide answers to questions received in response to the solicitation. Please see the attachment. This amendment also extends the due date for quotes by one (1) day to 10pm Eastern Local Time on Tuesday, 25 August 2015. The Uniformed Services University of the Health Sciences (USU) is the Nation's federal health sciences university and is committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense and the United States Public Health Service and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda. Additional information regarding USU can be found on our website: www.usuhs.edu. USU has a requirement for a fluorescence microscope for detecting protein crystals to be used in X-ray crystallographic structural determination. This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 - Acquisition of Commercial Items and FAR Part 13 - Simplified Acquisition Procedures. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-83-2-2-08032015 (effective August 5, 2015) and Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice (DPN) 20150626 (effective 26 June 2015). This requirement is set-aside for small business concerns under the applicable NAICS code 333314 Optical Instrument and Lens Manufacturing with a size standard of 500 employees. In order to be eligible for award, offerors must be registered in www.SAM.gov, with completed representations and certifications. A single firm fixed-priced (FFP) order will be awarded. Offerors are encouraged to submit multiple quotes for fluorescence microscopes which will be evaluated separately by the Government. Offerors responding to this requirement are not required to quote a fluorescence microscope meeting all of the desired specifications listed in the attached table; however, quotes meeting or exceeding more of the desired specifications may be considered more desirable by the Government. The selected offeror will be required to provide installation, training, and quote either a one-year extended warranty or a single option year of annual maintenance. The resultant award may include either a single year extended warranty or a single option year of annual maintenance, with the first option period beginning the day after the warranty expires. The Government intends to award a single firm-fixed priced (FFP) purchase order to the offeror representing the best value to the Government based on the evaluation factors listed below. Evaluation factors are listed in order of importance, with most important listed first. The Government will conduct a trade-off between price and non-price factors. The Government is not required to award to the lowest priced offeror or the offeror quoting the highest technically rated item. The Government reserves the right to award without discussions. Evaluation Factors (Listed in order of importance) : 1.Technical - Offerors must provide product literature, which demonstrates the capabilities of the quoted item(s). At a minimum the product literature must demonstrate how the quoted item meets the desired specifications outlined above. Information on warranty, training, installation and delivery lead-time must be included in the quote submission and will be evaluated. The Government reserves the right to utilize technical ( i.e., trade magazines) and customer reviews/references as part of the technical evaluation for the quoted equipment/supply. In addition to providing product literature, offerors must also submit the attached table with their quote submission. 2.Price - Offerors must include all applicable costs ( e.g., components, shipping, installation, training) and either the cost of a single year extended warranty or 12-month option period for maintenance in their quote. The total quoted price, inclusive of option period or extended warranty, will be the evaluated price. 3.Past Performance - The Government shall evaluate the offeror's present and past performance on similar tasks within the past three (3) years. Offerors must provide three (3) customer references for similar work performed. The Government reserves the right to select which reference(s) to contact. The following must be provided for each listed reference: a.Name of client and address b.Name of current point of contact including telephone number, e-mail address and title c.Contract number d.Contract value e.Contract period of performance f.A clear description of the supplies/services provided USU may request additional information at any time. Offerors will have an opportunity to address any adverse past performance history. Offerors who do not have any past performance history will not be rated favorably or unfavorably, but will instead receive a Neutral/Unknown rating for past performance. Offerors must clearly state in their quote if they do not have at least three (3) past performance references. Failure to provide this information may render an offeror's quote non-responsive and may not be considered for award. A strong history of relevant past performance may be more advantageous to the Government than a less relevant record of favorable performance. The Government reserves the right to use data provided by the offeror and data provided by alternative sources in conducting its assessment of past performance history. Offerors must also submit the following with their quote: •Completed copy of DFARS Provision 252.209-7992 •Completed copy of DFARS Provision 252.225-7000 •Completed copy of FAR Provision 52.222-48 See attached list for applicable provisions and clauses for this requirement. Quoted items must be new. Quotes for refurbished equipment will not be considered. Offerors must also indicate in quote where item is manufactured. Items will be shipped to Bethesda, MD. Deadlines : Questions/clarifications regarding this solicitation must be submitted via email to melissa.beale@usuhs.edu by 17 August 2015. Any question received after August 17th will not be answered. Questions will not be answered over the phone or submitted by any other means. All questions will be answered in an amendment to the solicitation posted on or around 19 August 2015. Quote packages are due by 10:00pm Eastern Local Time on 25 August 2015. Quotes must be submitted via email to melissa.beale@usuhs.edu. Quotes will not be accepted by any other means. Late quotes will not be considered. Quote packages should include a cover sheet that provides the following information: •Company Name •Point of Contact, to include email and phone number •DUNS Number •Statement that includes acknowledgement of latest amendment number
