Description
General Objectives and Requirements PURPOSE Contractor shall provide Healthcare for Homeless Veterans (HCHV) Services authorized by 38 U.S.C. 2031 and 38 CFR Part 63 and as outlined in VHA Handbook 1162.09. Location: Indianapolis, Indiana catchment area to support the Richard L Roudebush VA Medical Center (RLRVAMC) located at 1481 W Tenth Street, Indianapolis, Indiana 46202. Period of one (1) base year and four (4) option years, exercisable at the Government's discretion. The HCHV program was developed from the original Homeless Chronically Mentally Ill (HCMI) Program to a more foundational program with the provision of outreach services to Veterans who are homeless. The central goal of the HCHV program is to reduce homelessness among Veterans by conducting outreach to those who are the most vulnerable and who are not currently receiving VA services, and then engaging them in treatment and rehabilitation, as well as, in other VA programs and non-VA community programs that provide prevention and support services. This HCHV program is to remove homeless Veterans from the street and/or places not fit for human habitation and place them in community-based residential environments with sufficient therapeutic services to meet the needs of those Veterans. The Contractor shall provide community-based residential care, treatment and rehabilitative services for chronically mentally ill (CMI) veterans, with particular focus on those who are homeless or at imminent risk of homelessness. The Contract facility shall be located on a bus line and within 5 miles of the Richard L Roudebush VA Medical Center (RLRVAMC).
