- COMBINED SYNOPSIS/SOLICITATION FOR NON-EMERGENCY ADVANCED LIFE SUPPORT (ALS)/BASIC LIFE SUPPORT (BLS) GROUND AMBULANCE TRANSPORTATION SERVICE FOR THE NEBRASKA WESTERN IOWA HEALTH CARE SYSTEM (NWIHCS) FOR THE COMMUNITY BASED OUTPATIENT CLINICS (CBOCS) OF GRAND ISLAND AND LINCOLN NEBRASKA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested, and a written solicitation will not be issued. Quotes shall reference VA263-15-Q-0496. This Request for Quote (RFQ) document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83-2-2, dated August 3, 2015. The Sioux Falls Network Contracting Office 23 intends to award a firm-fixed-price, Requirements Contract for Non-Emergency ALS/BLS Ground Ambulance Transportation Service for the NWIHCS CBOCs of Grand Island and Lincoln Nebraska. The intent is to award a one-year base contract with two (2) additional 12-month option periods. The option periods may be exercised at the discretion of the Government. The North American Industrial Classification System (NAICS) code for this procurement is 621910, Ambulance Services. This is an unrestricted acquisition. All contractors must be registered in the System for Award Management (SAM) at https://www.sam.gov and registered under NAICS 621910 in order to be considered for contract award. The record must be complete and not missing elements such as representations and certifications. Questions concerning this solicitation shall be addressed to John Becker, Contracting Officer, and emailed to john.becker3@va.gov. All questions or inquires must be submitted in writing no later than August 26, 2015. No phone calls will be accepted. Any amendments to this solicitation will ONLY be available on the Federal Business Opportunities (FBO) website (www.fbo.gov). The Lowest Price Technically Acceptable (LPTA) source selection process will be used to evaluate quotes. The Government will award a contract resulting from this RFQ to the responsible contractor whose quote is the LPTA offer. To be Technically Acceptable, offerors shall submit required documentation as identified in this announcement and attachments. Quotes are due by 10:00 a.m. Central Standard Time, August 31, 2015. Quotes received after this day and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via e-mail to john.becker3@va.gov.
