- Introduction This is a SOURCES SOUGHT SYNOPSIS only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. No proposals or quotes are being requested or accepted with this announcement. No Telephone calls shall be accepted as a response to this notice. The Department of Veteran Affairs, Network Contracting Office 9, is seeking qualified Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, 8(a) firms, HubZone Business, and Small Business firms with the availability and capability of providing Mobile Lithotripsy Services onsite at the James H. Quillen VAMC located at the Corner of Lamont Street and Veterans Way, Mountain Home, TN 37684. NAICS code 621999 and size standard is $10.0 million is applicable to this procurement action. REQUIREMENTS Services Provided: Services shall include a minimum of one (1) up to two (2) Lithotripsy Technologist(s) (as needed) and Mobile Lithotripsy Equipment required to perform ultrasound shock wave treatments to dissolve kidney stones and other calculus formations for VA Beneficiaries at the JHQVAMC. Place of Performance: Contractor shall furnish services at the James H. Quillen VAMC located at the Corner of Lamont Street and Veterans Way, Mountain Home, TN 37684. Authority: Title 38 USC 8153, Health Care Resources (HCR) Sharing Authority. Term: The requirement is for a Base Year with four (4) one-year Option periods. QUALIFICATIONS TECHNOLOGIST License - Contract Technologist(s) assigned by the Contractor to perform the services covered by this contract shall have a current and unrestricted ARRT- certification/license in a State, Territory, or Commonwealth of the United States or the District of Columbia. Contract Technologist(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract. LITHOTRIPSY EQUIPMENT: Portable, state of the art, digital lithotripsy equipment that can be easily relocated to and from operating rooms at this facility. This includes all accessories, materials and supplies, including consumables (e.g., electrodes), as necessary to conduct seamless lithotripsy procedures with necessary equipment and at no additional cost to JHQVAMC. HOURS OF OPERATION VA Business Hours: The James H. Quillen VAMC facility is open 24 hours a day, 7 days a week, every day of the year. The VA anticipates that an average of five (5) Veteran beneficiaries will be treated per scheduled lithotripsy event. Work Schedule: Mutually agreed upon scheduled days per month selected from Monday through Friday, 8:00 a.m. - 4:30 p.m. (EST) Contract Technologists and Mobile Lithotripsy unit shall be readily available and present in the Urology Department during hours of responsibility at the James H. Quillen VAMC located at the Corner of Lamont Street and Veterans Way, Mountain Home, TN 37684, which will be established, and may be revised, as deemed appropriate for patient care by the Chief of Urology. RESPOND TO THIS RFI: Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set aside is possible for this requirement. Interested qualified Small Business firms responding to this announcement shall not exceed 5 pages and should include the following information: (1) A tailored capability statement indicating the firm's ability to provide Mobile Lithotripsy Technologist and Equipment to the location identified above; and (2) Name, address, phone #, and point of contract for your firm; and (3) The firm's DUNS Number; and (4) System for Award Management (SAM) Representations and Certifications document verifying an active SAM account (5) GSA Schedule Contract Number (if applicable) NOTE: 1. SDVOSB and VOSB must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. 2. GSA Schedule Contractor must provide its schedule contract number and contract end date.
