- Kennel Services Price Quote Sheet Vet Services Price Quote Sheet Statement of Work (SOW) Solicitation 1449 documentation. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is HSBP1015Q0188 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-83. The associated North American Industrial Classification System (NAICS) code for this procurement is both 541940 - Veterinary Services and 81290 - Pet Care, with a small business size standard of 500 employees. This requirement is a Small Business set-aside and only qualified offerors may submit bids. The US Customs and Border Protection has a requirement for DOG KENNELING AND VETERINARY SERVICES AT ORANJESTAD, ARUBA. The current requirement is for one dog, however, that number can fluctuate if dogs come and go for temporary assignments. Please see attached Solicitation Document and Statement of Work (SOW) for details of requirement. The solicitation pricing will start on the date this solicitation is posted and will end on September 4, 2015 at 1700 Eastern Time. FOB Destination shall be in the Place of Performance. The US Customs and Border Protection Agency intends to establish a single award Purchase Order to fulfill this requirement; however, the Government reserves the right to not award any order. The resultant Purchase Order award will be a single Base Year of 12 months, with four (4) 12-month Option Year periods. The Government anticipates award of a Purchase Order within 30 days of receipt of quotes. **Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered. *** Bid MUST be good for 30 calendar days after close of Buy. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Payment will be made by Electronic Funds Transfer (EFT). Please see attached solicitation documentation for full requirements and applicable clauses that must be read and complied with. Please complete and SIGN the price quote form(s) attached, and return with the signed SF 1449 Form. YOU MUST ALSO COMPLETE AND SIGN Blocks 13, 14, 15, and 16 on page one of the SF-1449 Form. Quotes that are incomplete, unsigned, or late will not be considered. Q&A -Please submit all questions directly to the Contracting Officer via email. This buy will then be reposted with Q&A based on the questions that come in (if applicable). Award Criteria-An award will be made to a responsive offeror (who submits all required submissions on time), whose past performance does not pose a risk to the Government, and whose offer is the Lowest Price Technically Acceptable (LPTA). An offer is technically acceptable if its technical capabilities conform to the Government's Statement of Work or listed specs whichever is applicable to the buy. In addition to providing pricing for this solicitation, each Offeror must provide any required, NON-PRICING responses (e.g. technical proposal, representations and certifications, etc.) so that they are received no later than the closing date and time for this solicitation.
