52.212-4 Contract Terms and Conditions - Commercial Items (May 2015) BBG50-Q-1073-15-00008 ATTACHMENT C BBG50-Q-1073-15-00008 ATTACHMENT A BBG50-Q-1073-15-00008 ATTACHMENT B Statement of Work This solicitation announcement is prepared and posted in accordance with Federal Acquisition Circular (FAC) 2005-83, Effective 03 Aug 2015. (FAR) Subpart 5.1 to notify potential Offerors for a competitive request for quote solicitation, issued by the Broadcasting Board of Governors, International Broadcasting Bureau (BBG). Combined Synopsis/Solicitation: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; request for quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a Request for Quotations (RFQ) under solicitation number BBG50-Q-1073-15-00008, for Non-Personnel Service. Award will be made using Streamlined Solicitation for Commercial Items with FAR Part 12. The Government intends to make a single contract award under FAR subpart Parts 12.6 and with a firm-fixed-price contract for services, and supplies. Description(s): The Broadcasting Board of Governors, International Broadcasting Bureau (BBG), Office of Contracts (CON), has an anticipated need for a contractor for: The BBG/IBB has designed a new HF dipole antenna to utilize this extra 250kW transmitter. This antenna will consist of one driven folded dipole element and one reflector element. It will be fed by an open-wire balanced transmission line which shall interface with an existing 250kW shortwave transmitter. Between them, a balanced to unbalanced (balun) transformer shall be installed. This contract will be for the fabrication and installation of an antenna system to implement this government-designed HF antenna system. Location: The IBB's Kuwait Transmitting Station (KTS) Kuwait City in Umm AI-Rimam, Kuwait. CLIN 001. The new antenna was designed by the BBG to operate between 5.8 and 6.0 MHz with an input power of 250kW. A third-party was contracted to design the foundation, guy anchors, and other mechanical details for the antenna structure (see Attachment C.) The contractor shall use this design to fabricate the tower bases and guy anchors for and erect the towers in the location indicated in Attachment A. The contractor shall also install the antenna elements based on the attached specifications. The government will provide the support towers, guy insulators, and antenna insulators as GFM. The contractor, however, is responsible for 1providing the remaining material and all labor required to install the antenna and its support structure. A preliminary parts Jist will be provided by the government as GFI, but the contractor shall provide all additional parts required for the antenna installation. The installation is being performed at a working transmitting facility. The contractor shall ensure that all work is performed in accordance with both the transmitting station's RF safety plan and the applicable guidelines for human exposure to non-ionizing electromagnetic radiation. The contractor shall address RF safety in its occupational safety plan submittal and ensure that all contractor personnel adhere to RF safety guidelines. Respondents are advised that the United States (U.S) Government will not pay for any information or administrative cost incurred in response to this Request for Quote Notice. All costs associated with responding to this Solicitation Notice will be solely at the responding party's expense. Please be advised that all submissions become Government property and will not be returned. All questions regarding this quote are due by September 1, 2015@ 8:00 am, eastern standard time (Washington, DC time on September 1, 2015) e-mail questions are to be sent to RFQsubmissionARW@bbg.gov Response must be received by BBG, no later than 10 September 2015, 2:00 p.m. Eastern time (Washington, DC time). This procurement is being issued as a 100% small business set-aside. The North American Industry Classification Systems (NAICS) code for this acquisition is 811310, with a small business size standard of $7.5 million. Request for Quotes must be submitted electronically to RFQsubmissionARW@bbg.gov EVALUATION FACTORS (Used to determine the competitive ranking of qualified applicants in comparison to other applicants. The factors are listed in priority order from highest to least.). The Government will award a Commercial Items, Firm-Fixed-Price, Non-Personal services contract resulting from this RFQ to the responsible offeror whose offer conforming to the RFQ will be most advantageous to the Government, based on : The evaluation factors that will be used to determine award will be (1) past performance, a minimum of three successfully completed projects in the last 10 years, that meets the qualifications listed in the Statement of Work (SOW) Requirements paragraph, (2) technical capability, and (3) price. For past performance Vendors shall provide three (3) relevant past performance references with their quote(s). Reference information shall include agency or business name, contract number, dollar amount, point of contact (include phone number and email address), for which your firm has provided the same or similar relevant services. Firms without a record of relevant past performance shall affirmatively state that no relevant past performance is available. Such firms will receive a neutral rating that will be evaluated neither positively nor negatively. Relevant past performance is defined as performance of contracts of similar size scope and complexity as this requirement performed within the past ten (10) years. The Government reserves the right to obtain and use past performance information from sources other than those listed in the quote, to include prior dealing with the government and personal knowledge of the company. All evaluation factors other than price, when combined, will be evaluated as significantly more important than price. Past performance, Technical, and price Basis for Award: Best value is defined as the expected outcome of an acquisition that, in the Government's estimation, provides the greatest overall benefit in response to the requirement, in accordance with FAR 2.101. Instructions to Offerors (a) Submission of offers. Submit signed and dated offers to the office specified in this request for quote at or before the exact time specified in this Request for Quote As a minimum, offers must show- (1) The Request for Quote number; Provide Dun and Bradstreet Number (DUNS) and a Commercial and Government Entity (CAGE) code or NATO Cage Code (NCAGE) number on your quote. If you do not have a DUNS and NCAGE, first register for DUNS at https://iupdate.dnb.com/iUpdate/viewiUpdateHome.htm. Next, register for NCAGE or CAGE at http://www.dis.dia.mil/Forms/Form_AC135asp(the instructions for US based organizations will take you to https://www.sam.gov/portal/public/SAM). (2) The time specified in the Request for Quote for receipt of offers; (3)The name, address, and telephone number of the Offeror; (4) Request for Quote, Offerors shall limit their response to: 14 or less pages. Technical, Past, and price shall be separate documents. Pages shall include: (a) Technical no more than 9 page(s) or less (b) 3 pages of past performance. (c) 2 pages pricing (3)Past performance information, to include 10 years of experience, at least 3 recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and Offeror qualifications shall include but not be limited to: (a) Past Performance: shall demonstrated past successful experience and installing guyed towers as well as the other requirements of C.4. Offeror's shall submit descriptions to include, but are not limited to: Descriptions of the work, project locations, and values in US dollars Names of site owners and their contact telephone numbers for Government verification A statement whether each project was completed within the original budget and schedule Quote must contain: a minimum of three successfully completed projects in the last 10 years that cumulatively include the following: The erection of guyed towers. Experience with installing overhead electrical service conductors and/ or Antenna radiators. Familiarity with RF Safety precautions when working near high power (>125kW) HF antennas. Experience with interpreting welding requirements from a drawing and executing them in a professional manner. A welding certification is required. (b) Technical capability: Offeror's Technical Knowledge and Approach Offeror's understanding of the requirements and the Offeror's descriptions of how the work will be planned and carried out. The descriptions shall include, but are not limited to: How to plan and carry out the work, Preliminary construction schedule, Major equipment to be used on sites, and Whether and how sub-contractors might be employed. Organization and Project Management Approach: The Descriptions shall include, but are not be limited to: Safety and personal injury prevention plan and RF safety plan. Project management and installation quality control plans. Names, experience, and qualifications of the project managers, welders, and the on-site supervisors. Company names of all subcontractors and the descriptions of their intended services or deliverables. (c) Price All Requests for Quote as well as inquiries should be sent electronically, to Adrienne R. Washington NO PHONE CALLS WILL BE ACCEPTED. All quotes must be in English, and US currency. OFFERORS MUST BE REGISTERED WITH SYSTEM FOR AWARD MANAGEMENT (SAM) PRIOR TO CONTRACT AWARD. The addendum is as follows: CONTRACTING OFFICER REPRESENTATIVE: The Contracting Officer will appoint by letter a Contracting Officer Representative (COR) who will have the responsibility of ensuring that the work conforms to the requirements of the contract and such other responsibilities and authorities as may be specified in the letter of authorization or this contract. It is understood and agreed, in particular, that the COR shall not have authority to make changes in the scope or terms and conditions of the contract unless and only to the extent that such authority is specified in the letter of authorization or the contract. THE RESULTANT CONTRACTOR IS HEREBY FOREWARNED THAT, ABSENT THE REQUISITE AUTHORITY OF THE COR TO MAKE ANY SUCH CHANGES, IT MAY BE HELD FULLY RESPONSIBLE FOR ANY CHANGES NOT AUTHORIZED IN ADVANCE, IN WRITING, BY THE CONTRACTING OFFICER, MAY BE DENIED COMPENSATION OR OTHER RELIEF FOR ANY ADDITIONAL WORK PERFORMED THAT IS NOT SO AUTHORIZED, AND MAY ALSO BE REQUIRED, AT NO ADDITIONAL COST TO THE GOVERNMENT, TO TAKE ALL CORRECTIVE ACTION NECESSITATED BY REASON OF THE UNAUTHORIZED CHANGES.
