This is a combined synopsis/solicitation amendment for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The purpose of this amendment is to extend the closing date of this RFQ to allow the government time to properly research the questions submitted and provide adequate responses to the questions submitted. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-15-Q-0378 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. The associated NAICS code is 334516 Analytical Laboratory Instrument Manufacturing and small business size standard is 500 employees. This is a total small business set-aside. The requirement is for a Reverse Osmosis Deionized Water Purification System. Delivery location is the Louis Stokes, Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106-1702. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-82. STATEMENT OF WORK (SOW) STERILE PROCESSING SERVICE Reverse Osmosis Purified Water System Background: The Sterile Processing Service (SPS) requires a water purification system that produces Clinically Treated Water as defined by the Association for the Advancement of Medical Instrumentation (AAMI) Guideline ST-79/TIR-34 and the Clinical and Laboratory Standards Institute Water standards. The current system has become inadequate due to the recent move of the laboratory's Clinical Water System, which SPS was tapped into, and the RO machine currently connected to Washer Decontaminators does not meet AAMI ST-79/TIR-34 standards. This equipment is essential in the final rinse of machine and hand washed instrumentation. It prevents staining and contamination of instrumentation, reduce residuals on surgical instrumentation which can affect disinfection and sterilization efficacy and/or cause adverse reactions in subsequent patient use. Clinically Treated is mandatory in order to prevent Toxic Anterior Segment Syndrome (TASS) while processing intraocular surgical instruments. Objectives: The contractor will provide and install a reverse osmosis water purification system that can produce Clinically Treated Water (Deionized, Distilled, Reverse Osmosis). The equipment shall produce purified water according to Association for the Advancement of Medical Instrumentation (AAMI) ST-79/TIR34 and Clinical and Laboratory Standards Institute (CLSI) specifications and the specific criteria listed in the table. In an appropriate Central Supply water system per TIR34, water must be softened, deionized, and high-purity water treated. ST-79, TIR-34, and Manufactuer IFU must be consulted in order to ensure compliance and provide the best quality outcome as possible. Tasks: "The contractor shall install a purified water system in basement of the Cleveland VA Medical Center, Wade Park, room 127. "Installation will include hookup to 4 washer decontaminators, 3 ultrasonic Machines, 3 stand-alone sinks, 1 cart washer, and a free standing line to room 116 (Scope Room) for future utilization. "At time of installation, contractor will supply a system manual, system process flow diagram, and system log sheet. Contractor shall also provide at least one training session on the system operation/maintenance to SPS staff and one training session to Engineering staff. Salient Characteristics: Please consult AAMI TIR-34 Table 1 for requirements for water quality used in medical device reprocessing. "System shall provide purified water according to (AAMI) ST-79/TIR34 "System will make up to 300 gph into a minimum sized 240 gallon holding tank with dish or conical bottom food grade construction tank to have hydrophobic submicron vent filter. "R/O system to operate at 75% recovery to conserve water "R/O to use CRN series Grundfos pump "Exchange Deionization tanks will purify water to be regenerated at a facility that follows ISO Certification for the regeneration process. "Loop delivery pump shall be variable speed design with integrated pressure sensor to automatically adjust speed to various flow needs Grundfos CRNE "Pump to be capable of a maximum 30 gpm "A UV light and.2 submicron filtration will be used in the recirculation loop and properly sized for the recirculation loop flow rate "Loop to be constructed of hi purity polypro and installed per (AAMI guidelines in ST-79/TIR34) "Digital temperature compensated resistivity meter will be used to monitor resistivity in loop "Loops to have shut off valves at each point of use outlet "Loops and tank must be able to be sanitized with Chlorine Dioxide. "Provide any and all water quality testing materials "1 year Service package with option years will include oSystem water analysis, oBacteria and endotoxin samples collected with results provided to the VA after water testing is completed. oSanitize unit with Chlorine Dioxide per Manufacturer instructions oExchange of DI and Exchange carbon filters when needed oMonitoring of any filters and water treatment equipment per TIR 34, Table B.1. If this is captured electronically, all data must be able to be obtained by SPS or Engineering personnel. Vendor Requirements "Work shall be accomplished during night and weekend hours in order to facilitate patient care. "Phased installation may be required in order to keep the SPS department open for patient care during renovation. SPS is routinely closed except to emergency cases on weekends and holidays. Work performed during normal working hours and off-shift hours must be coordinated through the Chief/Assistant Chief of SPS. "Vendor shall install temporary barriers around work areas and maintain acceptable separation between clean and dirty areas of SPS area during equipment replacement. "Vendor must provide and make utility connections to all new units. "Vendor is responsible to verify existing utilities correspond with utility requirements of proposed equipment. Proposed equipment with differing utilities shall be modified by the vendor to provide a turnkey installation to the Louis Stokes Cleveland VA Medical Center, Sterile Processing Service. "Contractor must verify all existing field conditions prior to providing a proposal, and is required to attend a scheduled pre-bid site inspection prior to submission of proposal. Vendor assumes the responsibility of surveying the existing conditions as part of their performance. "The installation scope of work includes all equipment, supplies, and labor required for complete turnkey installation of the unit within SPS. "Vendor to remove old SPS water system components "Vendor to install and plumb wire new system including A temperature blending valve and backflow device, floor drain, and all connections to and from hot and cold water supplies and sterilization/decontamination equipment. "Tie ins to new equipment to be done on off hours or when scheduled by VA "System water analysis, bacteria and endotoxin samples collected after startup and results will be provided to the VA after water testing is completed. "System started up and all set-points adjusted for proper operation "Provide a minimum of One two hour in service on system operation and training for SPS and engineering staff "Provide a minimum of One process and instrumentation drawing provided in pdf format "Vendor to include freight and shipping VAMC Cleveland Requirements "VA to provide electrical disconnects 3 phase one for R/O unit and one for recirculation pump "VA to provide 110 vac power to control panel and 110 vac outlet power Specific Water requirements for Steris Vision washers "This equipment utilizes a total of 28 gallons of water per cycle x 4 washers = average of 224 gallons of water per hour (GPH) used. DI/RO water must be provided via 1/2" FPT at a flow of 10 Gallons per minute (GPM) to each washer = a total of 40 GPM for all washers in total. "Pre-Wash phase - o Cold or cold and hot water from building supply fills the chamber. The water is recirculated through the rotary spray arm assemblies for a selected time period. Water is drained upon phase completion. Up to four Pre-Wash phases can be selected per customized cycle. The temperature of the water used to pre-clean soiled instruments should not exceed 45 ºC (113 ºF). "Wash phase - o Cold and/ or hot water from building supply fills the chamber while Enzymatic detergent is automatically added at the beginning of the phase. The phase solution is recirculated through the rotary spray arm assemblies for a selected time period. If heated water is selected, water is heated by a steam or electric coil in the sump. Solution is drained upon phase completion. Up to four Wash phases can be selected per customized cycle. Provided that the water meets the enzymatic detergent manufacturer's criteria and has the characteristics described in Table 1, the water used for the enzymatic prewash stage may be potable, softened, or deionized. High-purity water is not necessary for this stage of cleaning. oSecond stage is selected, cold and/ or hot water from building supply fills the chamber at phase beginning. Load is washed for a selected time period (factory setting) with enzymatic cleaner-injected heated water (factory setting). The detergent manufacturer's instructions should be consulted before softened, deionized, or high-purity water is used in main detergent wash stages. oAfter the selected time period, neutral detergent is injected while injected water is heated (factory-setting). Then, load is washed for a selected time period with both chemicals. Solution is drained upon completion. Up to four Wash phases can be selected per customized cycle. "Rinse phase - oHot or pure water from building supply fills the chamber. The phase solution is recirculated through the rotary spray arm assemblies for a selected time period to rinse load and to cool chamber. Solution is drained upon completion. Up to four Rinse phases can be selected per customized cycle. "Thermal Rinse phase - oHot or pure water from building supply fills the chamber. If needed, instrument lubricant or rinse aid is automatically added during the filling at phase beginning. This solution is recirculated through the rotary spray arm assemblies for a selected time period. Water is heated by a steam or electric coil in the sump. Water can be heated from 180 to 194 °F (82.2 to 90 °C). Solution is drained upon completion. Only one Thermal Rinse phase can be selected per customized cycle. For critical devices, high-purity water may be considered because of concerns about specific water contaminants (e.g., endotoxins). For example, as noted in 3.2.3.1 of TIR-34, manufacturers of ophthalmic surgical instruments recommend that devices used for cataract surgery be thoroughly rinsed with sterile distilled water (i.e., water equivalent to the high-purity water described in Table 1) before sterilization. The maximum temperature of the rinse water should be compatible with the items being processed. Many medical devices are temperature-sensitive and can be damaged if the rinse temperature is too high. Specific Water Requirements for Steris Caviwave Plus Ultrasonic Cleaners "This equipment has a load capacity of 17 Gallons of water per cycle x 3 units = Average use of 102 gallons of water per hour. "This equipment must have DI/RO water provided via 1/2" FPT (38 psi Maximum); 65-120 °F (18-49 °C) at a flow of 6 Gallons per Minute (GPM) to each ultrasonic. This will result in a total flow of 18 GPM for all ultrasonic cleaners. Specific Water Requirements for Steris Reliance 1227 Cart washer "This equipment has a load capacity of 50 gallons of water per cycle. Average use of 100 gallons of water per hour. "Per Steris, there is not a DI/RO water connect on this unit. "Wash phase. Hot water enters the sump or the reservoir (if option is present). A measured amount of detergent is added automatically to the water. This wash solution is recirculated through the spray system for the selected time period. Solution is not heated during the first portion of the washing phase and heated to 135 °F (57 °C) during the second portion. At phase completion, water is pumped to the drain. "Thermal Rinse phase. Hot water enters the sump or the reservoir (if option is present). A measured amount of rinse aid is added automatically to the water. Rinse solution is sprayed over load and recirculated for the selected time period to rinse and disinfect load. Load is heated to 176 °F (80 °C). At phase completion, water is pumped to the drain. Specific Water Requirements for Steris triple bay sinks "This equipment has a load capacity of 10 gallons of water used per sink per hour. Therefore for each triple sink approximately 30 gallons of water can be used per hour. This will result in a 90 gallon of water use of water per hour. "Each sink with utilize a flow of 2.2 Gallons of water per minute. This results in a total of 6.6 GPM for all sinks. Delivery: Delivered to the Louis Stokes Cleveland VA Medical Center 10701 East Boulevard, Cleveland, OH 44106-1702 Security Requirements: The contractor will need to be granted physical access to room 117 and 127 to install and test the water purification system. There are no Information Security elements related to this procurement. At least 48 hours notice prior to requiring access must be given to ensure access is not delayed.
