- A. Small Business Sources Sought Notice: NHLBI-CSB-DE-2015-239-KEW-SBSS B. Title: Dental Public Health Residency Program Manager C. This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified business sources; and (2) their size classification relative to the North American Industry Classification System (NAICS) code 541690 for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method. FAXES ARE NOT ACCEPTED D. Information: 1. Background Dental Public Health is one of the nine subspecialties of dentistry. For 35 years NIDCR has sponsored an accredited residency training program in Dental Public Health. The DPH Residency Program provides a formal training opportunity for dentists planning careers in dental public health, with an emphasis on oral and craniofacial, health-related epidemiologic research. The Residency Program is accredited by the Commission on Dental Accreditation of the American Dental Association. Program graduates receive a certificate of completion and are qualified to apply for examination by the American Board of Dental Public Health for specialty certification. While emphasizing research training and oral disease prevention and health promotion, the residency also provides experience in other areas of dental public health, i.e., public health administration and management, the organization and financing of dental care programs, and the development of resources. Residents develop an individualized initial training plan, which describes activities to be undertaken during the residency and conduct at least one research project under the guidance of NIDCR staff and other qualified mentors. The Commission on Dental Accreditation evaluates the educational quality of dental specialty programs in the United States, including the NIDCR DPH Residency program. The CODA review consists of a rigorous and comprehensive examination of documentation regarding the operations of the DPHR program, followed by an on-site review process to obtain in-depth information concerning all administrative, educational, training, and personnel components of the program. In addition, the site visit permits a team of Commission-appointed peers to assess a program's compliance with the Accreditation Standards and with its own stated goals and objectives. The site visit verifies and supplements the information contained in the comprehensive self-study document completed by the institution. The CODA review takes place every seven years, and 2015 is a review year for the NIDCR DPH Residency program. The site visit report, along with the institutional response to the report, serves as the Commission's primary basis for accreditation decisions. 2. Purpose and Objectives To provide professional support services for the NIDCR Dental Public Health Residency (DPHR) program, and to provide direct support to its faculty and Dental Public Health (DPH) residents. The objective is to procure continuity of administrative, managerial, logistical, and educational support for the DPHR program, in order to: 1) Qualify for, apply for, and successfully complete the upcoming required accreditation review of the DPHR program by the American Dental Association's Commission on Dental Accreditation (ADA CODA); 2) Maintain and enhance the quality, professionalism, and training effectiveness of the DPHR program.  3. Project Requirements Scope of work and tasks for Dental Public Health Residency Program Manager: • Assume primary responsibility for the DPH residency program's preparation for the November 2015 review by the American Dental Association Commission on Dental Accreditation (ADA CODA). This includes completion of the DPH residency program's application to the ADA CODA, compilation of supporting materials; final preparation of all application materials for submission to the ADA CODA; preparation and orientation of NIDCR staff prior to the CODA site visit; and assisting CODA examiners during their site visit. • Prepare as necessary the response to the DPH residency program's CODA evaluation, and assist with any post-review requirements and remediation's, until an unconditional program accreditation is received. • Develop the DPH residency program's policy and procedures manuals, including the Orientation Manual for residents and the Operations Manual for the program. • Coordinate and participate in the annual application review, interview, and selection process of DPH residents. • Take an active role in residency mentoring, instruction, and curriculum development; attend residency-related meetings, and organize activities such as DPH journal club, instructional webinars, site visits, and seminars. • Participate in and document key DPH residency meetings and decisions. • Provide managerial and administrative assistance to the DPH residency director, including DPH Residency program record keeping; curriculum planning; logistical support for residency activities; evaluation of residency candidates; and maintaining communication and collaboration with other residency and fellowship programs. • Serve on the Residency Training Advisory Committees of all DPH residents. • Systematize and refine the record keeping and documentation for the residency program. • This position requires a time commitment of two (2) full days (8 hours) and one (1) half day (4 hours) per week. 4. Anticipated Period of Performance/Delivery Date Base Year: October 1, 2015 - September 30, 2016 Option Year 1: October 1, 2016 - September 30, 2017 Option Year 2: October 1, 2017 - September 30, 2018 Option Year 3: October 1, 2018 - September 30, 2019 Option Year 4: October 1, 2019 - September 30, 2020 5. Capability Statement Interested parties are expected to review this notice to familiarize itself with the requirements of this project. Failure to do so will be at your firm's own risk. The following information shall be included in the capability statement: a. A general overview of the respondents' opinions about the difficulty and /or feasibility of the potential requirement, and any information regarding innovative ideas or concepts. b. Information as needed in sufficient details of the respondents' (a) staff expertise, including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. c. The respondents' DUNS number, organization name, address, point of contact, and size and type of business (e.g., 8(a), HUBZONE, etc) pursuant to the North American Industry Classification System (NAICS) code: 541690, Other Scientific and Technical Consulting Services, Small Business Size $15 Million. d. Any other information that may be helpful in developing or finalizing the requirements of the potential acquisition. e. The capability statement shall not exceed 20 single-sided pages (including all attachments, resumes, charts, etc.) presented in single-space and using a 12-point font size minimum, in either Microsoft Word or Adobe Portable Document Format (PDF), with 8-1/2 by 11 inch paper size, and 1 inch top, bottom, left and right margins. f. All proprietary information should be marked as such. Statements should also include an indication of current certified small business status; this indication should be clearly marked on the first page of your capability statement (preferably placed under the eligible small business concern's name and address). Responses will be reviewed only by NIH personnel and will be held in a confidential manner. 6.
