This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W9114F-15-T-0088 is issued as a Request for Quotation (RFQ). Equal products of the brand name manufacturer must meet the following equipment technical factors: SLEEPER CHAIR: Factor 1: The sleeper chair is used for the spouse of significant other of patients receiving medical care on the maternal child wards. The sleeper must be used for overnight visitation within hospital setting. Factor 2: Sleeper chair must provide adequate amount of comfort for the family member who stay with maternal child patients. Maximum width of 37 inches, maximum height of 32 inches, length will be adjustable from 35 inches to 89 inches. The mattress must be reasonably sized to fit an adult. The mattress should be a single mattress, no less than 72 inches in length. Factor 3: The sleeper chair must be upholstered with a durable and cleanable material such as vinyl. Vinyl is durable and impervious to fluids and stains. Vinyl is resistant to scratches and does not damage easily. Vinyl is also hypoallergenic. Factor 4: The sleeper chair will require locking caster wheels to ensure chair can be moved from one area to another and lock will provide stability while in use. Factor 5: The overall construction of the bed must be able to support an overall weight of 136kg during sleep use. Construction materials must meet flammability standards of CAL 117. Factor 6: The mattress must have a fire barrier cover that meets the 16 CFR 1633 requirements. Factor 7: The delivered equipment shall be new and not a used or re-manufactured product. Factor 8: The sleeper chairs must be warranted to be free from material and workmanship defect for 1 year. Factor 9: Contractor must meet all salient characteristics. HIGHBACK CHAIR 21 quote mark Factor 1: The high back patient chair is used for the spouse, significant other, or family members of patients receiving medical care on the maternal child wards. The high back patient chair is used to provide a place for the family member to sit, visit with, and hold newborn infants in relative comfort and stability. Factor 2: The high back patient chair must provide adequate amount of comfort and stability for the family member who visit with maternal child patients. Maximum width of 24 inches, maximum overall height of 44 inches, length will be 26 inches. The weight should be no more than 24 pounds. Factor 3: The high back patient chair must be upholstered with a durable and cleanable material such as vinyl. Vinyl is durable and impervious to fluids, microbial resistant, mildew resistant, oil resistant, cold crack, and stain resistant. Vinyl is resistant to scratches and does not damage easily. Vinyl is also hypoallergenic. Factor 4: The overall construction of the chair must be able to support an overall weight of 136kg during sleep use. Construction materials must meet flammability standards of CAL 117. Factor 5: The chair should meet safety standards prescribed in ANSI/BIFMA X5.1-2002 requirements for office seating. Factor 6: The delivered equipment shall be new and not a used or re-manufactured product. Factor 9: The high back patient chairs must be warranted to be free from material and workmanship defect for 1 year. Factor 9: Contractor must meet all salient characteristics. TRADITIONAL BASSINET Factor 1: The bassinet is used for newborn patients receiving medical care on the maternal child wards. The bassinet is used to provide a place for newborn infants to rest in relative comfort and stability. Factor 2: The bassinet must provide adequate amount of comfort and stability for the newborn infant while staying on the maternal child ward. Maximum length of 38.5 inches, maximum overall height of 42 inches, and the overall width will be no more than 21 inches. The weight should be no more than 100 pounds. Factor 3: The construction of the bassinet should provide for a counter-weight balance base to ensure stability of bassinet. Factor 4: The bassinet should be constructed of wood and finished with a finish that provides a surface for durability and cleanliness. Factor 5: The bassinet should have large caster wheels to ensure mobility of bassinet without instability and provide a locking mechanism on the caster wheels to ensure newborn patient is secure when unattended. Factor 6: The delivered equipment shall be new and not a used or re-manufactured product. Factor 7: The high back patient chairs must be warranted to be free from material and workmanship defect for 1 year. Factor 8: Contractor must meet all salient characteristics. ADENDUM TO FAR 52.212-2 The Government will issue a Purchase Order to the quoter whose quote represents the best value to the Government, price and other factors considered. Award will be made on the basis of Lowest Price, Technically Acceptable (LPTA) quote. If any Technical requirement below is rated quote mark Unacceptable, quote mark then the Government will no longer consider issuing a Purchase Order to that quoter, thus the quoter will be ineligible for award. EVALUATION STANDARDS. The quoter's technical quote will be evaluated using the following standards for a final rating: ACCEPTABLE: A quote that offers a product which meets all technical requirements identified in the solicitation. UNACCEPTABLE: A quote that offers a product which fails to meet one or more of the technical requirements identified in the solicitation.
