- SOW Davis Bacon Act Wages for Cuyahoga County, Ohio found at www.wdol.gov SF 1413 from http://www.gsa.gov/portal/forms/download/115794 Serrato Form 542 Vendor Set up form 502 THIS IS A SUBCONTRACTING OPPORTUNITY Request for Proposal A. General a. Type: This is a subcontracting opportunity for store front door replacement and repairs. b. Agency/Office: Cleveland Job Corps Location: 13421 Coit Road Cleveland, Ohio 44110 c. Site Visit Date : September 8th at 10:00am est d. Closing Date: September 14th at 4:00pm est B. Period of Performance: Septemeber to October 2015 C. Statement of Work Project Title: Deficiency 87122 Exterior Doors & Frames Background: Exterior front doors on the three dormitory buildings (Buildings F, G and H) and two doors on the student trade building (Building B) do not close properly. Two man doors on Building D do not have windows. Scope of Work: The contractor shall remove existing exterior front doors and frames from G, H, F two of the five doors from Building B (B2 and B4) and dispose of them. The existing doors are 3'x8' and are to be replaced with 3'x7' The doors to G, H and F are double doors with a removable center post The Contractor shall remove the current hollow metal man doors of Building D from both sides of the dock and replace these doors with new hollow metal doors containing a 2'x2' windows. The Contractor will provide replacement doors. These doors shall be of a heavy duty metal frame door with windows that are to withstand high winds and heavy traffic. Replacement doors are to be at same grade or gauge of material in its place or greater equivalent of the YKK Ap's Proteck Ap Hurricane and Blast Solutions or Kawneer 350/Tuffline entrance doors. The Contractor will have the doors painted to match existing doors and windows on center with the Colonial Red by Valspar Corporation. The Contractor will provide and install new hinges, closers and thresholds which will withstand high winds such as the door closure Sargent 351 Series Aluminum closer or equivalent with appropriate metal screws, and a continuous roton hinge that will handle high traffic and can withstand strong winds. Total Labor b. Material c. Other d. Total Price F. Evaluation Criteria: To be considered for award, a bid must comply in all material respects with the invitation for bid. Award will be made to that responsible bidder whose bid, conforming to the invitation, is most advantageous to Serrato Corporation, considering only price and the price-related factors includined in the invitation.
