Description
Statement of Work (I) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 15-236-SOL-00035 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. (iv) The North American Industry Classification System (NAICS) code is 611430 with a size standard of $10 Million. The Indian Health Service (IHS), Division of Acquisition Policy intends to solicit a firm fixed priced purchase order to Small Businesses only. (v) The Contract Line Items (CLINs) and pricing structure are as shown below. CLIN0001: A total package for on-line training of all personnel for the base year (9/29/15-9/28/16) - unlimited access for continuing education training modules for the IHS nursing staff. UNIT: 1 YEAR tiny_mce_marker_____________ CLIN0002: A total package for on-line training of all personnel for the option period 1 (9/29/16-9/28/17) - unlimited access for continuing education training modules for the IHS nursing staff. UNIT: 1 YEAR tiny_mce_marker_____________ CLIN0003: A total package for on-line training of all personnel for the option period 2 (9/29/17-9/28/18) - unlimited access for continuing education training modules for the IHS nursing staff. UNIT: 1 YEAR tiny_mce_marker_____________ CLIN0004: A total package for on-line training of all personnel for the option period 3 (9/29/18-9/28/19) - unlimited access for continuing education training modules for the IHS nursing staff. UNIT: 1 YEAR tiny_mce_marker_____________ (vi) The IHS is in need of access to on-line continuing education training modules in order for nurses to maintain certification. This includes the 36 pre-existing Public Health Nursing (PHN) Training modules for which IHS will be provide the required information. Users shall be able to participate in self-directed Web-based education which shall be available at all times. Staff shall be able to decide when to access the training, day or night, from their home or while they are at work. The training modules must offer accreditation for continuing nursing education credits by American Nurses Credentialing Center (ANCC). **See the attached scope of work. (vii) The delivery and acceptance term for this order is FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Apr 2014), applies to this acquisition and a statement regarding any addenda to the provision (ix) 52.212-2, Evaluation - Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government intends to evaluate offerors and award a contract without discussions with offerors. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if later it is determined by the Contracting Officer to be necessary. The Government may reject any or all offers, waive informalities and minor irregularities in offers received. The following factors will be used to evaluate offers: Technical Capability, Relevant Past Performance & Price. Technical Capability and Past Performance, when combined, are more important than the price. The detailed evaluation criteria are listed below: Technical Approach -Technical Proposals shall be limited to fifteen (15) pages and must provide the following information: 1) The contractor shall be accredited by the American Nurses Credentialing Center (ANCC) and provide documentation. 2) Contractor shall provide a list of training modules to be provided to IHS users. 3) Contractor shall provide methodology to publish training modules on contractor's website. 4) Contractor shall describe their approach to work with IHS staff for training modules to be updated. 5) Contractor shall provide documentation that evidences all functionalities listed under the scope of work section 2 can be met by capabilities of the proposed product. 6) Contractor shall provide a detailed process of how they will create new or custom educational modules including use of subject matter experts to successfully develop the material. 7) Contractor shall describe their approach to include identified existing evidenced based/standardized training modules. Past Performance - Past Performance references must be limited to three (3) pages and provide the following information: 1) The contractor shall show a proven track record or sufficient staff competence and capability in executing the requirements the Scope of Work. 2) Offeror shall demonstrate their understanding of IHS mission, goals and customers. It is important for the contractor to understand the health care system of the Agency. (x) FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Mar 2015) - The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (p) of this provision. (xi) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (May 2015) applies to this acquisition.
