Northern Arizona VA Healthcare System (NAVAHCS) STATEMENT OF WORK -Radiology System Upgrade Purpose: Purpose of this project is to upgrade our radiology software/operating system. Our current Planmeca software/legacy program is Windows XP compatible, but all of our computer operating systems have been upgraded to Windows 7. The differences in workstation operating system and software are causing multiple difficulties/problems including loss of critical digital images. Consequently patient care will be comprised if the software is not upgraded. Specialty and Diagnostics, Dental Department at NAVAHCS prides itself on providing high quality services to our Veterans and therefore recommends that the required software be purchase and installed. 1.Requirements: Equipment requirements are as follows for the Planmeca imaging software: A.Operating system: Windows Vista Pro/ Windows 7/Windows 8 B.Image Formats DICOM (2D and 3D image). Actual specifications are submitted, enclosure 1 C.Installation options Client Server/Java Web Start deployment D.Minimum set up: Client workstation and database server; Windows 2008 or Windows 2012 platform server and Planmeca Romexis image Database E.Interface: PMBridge (patient information and images) F.ProMax Ethernet kit with CPU board G.The most optimal software choice will communicate between the Planmeca x-rays, VistA Imaging, DRM Plus and CPRS H.New equipment only. No refurbished products. Items must be in original packaging, never used, and not altered in any way I.Components of requested equipment includes: i.ProMax Ethernet kit with CPU board ii.C-Arm Control Module PCB, C-Arm main cable iii.Install Ethernet kit and Recal Promax iv.10 - Simultaneous Users/Romexis License for 2D v.DICOM Software License J.All items must be covered in the manufacturer's warranty and procured through a manufacturer approved distribution channel. Sellers must be able to document their ability to provide items through manufacturer approved distribution channels upon request. 2.The Seller confirms to have sourced all products submitted in this Bid and or VA contract from manufacturer approved channels for Federal sales, in accordance with all applicable laws and manufacturers' current applicable policies the time of purchase. Seller must be able to support both the product sale and product pricing, in accordance with applicable manufacturer certification/specialization requirements. 3.Records Management Language for Contracts: The following standard items relate to records generated in executing the contract: A.Citations to pertinent laws, codes, and regulations such as 44 U.S.C. chapters 21, 29, 31, and 33; Freedom of Information Act 5 U.S.C. 552; Privacy Act 5. U.S.C. 552a; 36 CFR Part 1222 and Part 1228. B.The Contractor shall treat all deliverables under the contract as property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. C.The Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. D.The Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. E.The Contractor shall not create or maintain any records containing any government Agency records that are not specifically tied to or authorized by the contract. F.The Government Agency owns the rights to all data/records produced as part of this contract. G.The Government Agency owns the rights to all electronic information and all supporting documentation created as part of this contract. The Contractor must deliver sufficient technical documentation with all data deliverables to permit the Agency to use the data. H.The Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format, mode of transmission, or state of completion. I.No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage, or alienation of Federal records is subject to the files and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. J.The Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship in support of this contract requiring the disclosure of information, documentary material, and/or records generated under or relating to this contract. The contractor is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. 4.Specific Tasks: A.The Contractor shall notify either Biomedical Engineering or the Dental Department of his/her intent to come on-site before arrival or before remoting into the Planmeca workstation. B.The Contractor shall check-in with the VA Police Department upon arrival, before doing any work on-station. C.The Contractor shall install the new system D.The Contractor shall transfer old data to the new system. E.The Contractor shall deliver a hard copy or electronic copy of a Field Service Report to Biomedical Engineering that delineates the service performed and confirms quality assurance tests results. F.The Contractor shall not remove any hard drives off station. All hard drives will be turned in to Biomedical Engineering. 5.Performance Monitoring: The Imaging Department and Biomedical Engineering will oversee work done by the Contractor and accept the quality of work as appropriate. 6.Other Pertinent Information or Special Considerations: None. 7.Identification of Possible Follow-on Work: None 8.Identification of Potential Conflicts of Interest (COI): None 9.Identification of Non-Disclosure Requirements: The Contractor will have physical and remote service/administrative access to the equipment. This may allow the Contractor to view patient information. 10.Inspection and Acceptance Criteria: The end user will determine if the system has been set up properly and notify the COR of any issues. 11.Risk control: None 12.Delivery Date: ASAP after order is placed 13.Delivery Location: Prescott Northern Arizona VA Health Care System (Station 649) 500 Hwy, 89 N Prescott, AZ 86313 Warehouse 14.Point of Contact: 15.Delivery Charges: All delivery charges must be included in the price of items. 16.Technical Support: Technical training is free to Biomedical Engineering personnel. 17.Security accreditation package is required: There is a business associate agreement between the Department of Veterans Affairs Veterans Health Administration and Planmeca USA INC. 18.Place of performance: Northern Arizona VA Health Care System A.Building 155, Room 117 B.Building 12, Room 109. 19.Type of Contract/Payment: Net 30 unless otherwise negotiated.
