Form 1449 / Model Contract Performance Work Statement THIS SOLICITATION REPLACES SOLICITATION NUMBER FA862315CC6413 POSTED IN ERROR. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Solicitation Number is FA8623-15-R-6413 and is issued as a request for proposal (RFP). The Air Force Life Cycle Management Center (AFLCMC) Acquisition Environmental and Industrial Facilities Division (WNVK) intends to award a competitive Proposal for 2015 Inspect and Test Tanks at AFP-42, Common Area (Palmdale, CA). The National American Industry Classification System (NAICS) Code is 562998, with a Small Business size Standard of $15,000,000. The Solicitation Document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 205-79. This requirement is a (Small Business) 100% set-aside and only qualified vendors may submit bids. This Combined synopsis/ Solicitation will begin when posed on www.fbo.gov and end on 2015.09.11 14:30:00.0 Eastern Time or as otherwise displayed at www.fbo.gov. Attached please find the Air Force Performance Work Statement (PWS) and attached form 1449. ### Please Complete and return the attached SF1449 as part of your proposal. ***Question Submission: Interested Offers must submit any questions concerning this solicitation at the earliest possible time to enable the Buyer to respond. The selected Offeror must submit a completed copy of the provision at 52.212-03, Offeror Representations and Certifications - Commercial Items. FAR 52.212-04, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The selected Offeror must comply with the following commercial item terms and conditions: FAR 52.209-05, Certification Regarding Responsibility Matters and FAR 52.212-01, Instructions to Offerors - Commercial, applies to this acquisition. The selected Offeror must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. FAR 52.219-6, Notice of Total Small Business Set-Aside, applies to this acquisition. 52.212-2, Evaluation - Commercial Items (Oct 2014), applies to this acquisition. Any resulting order will be issued to the company offering the best value to the Government, price and other factors considered. The following FAR clauses in paragraph (b) of FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, will apply: 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.225-13, Restriction on Certain Foreign Purchases; and 52.232-33, Payment by Electronic Funds Transfer Central Contractor Registration. The following DFARS clauses are applicable under DFAS 252.212-7001: 252.225-7001, Buy American Act and Balance of Payments Program; This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Bid MUST be valid for 30 calendar days after submission In accordance with DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports, the contractor shall submit their payment request electronically using the Wide Area Workflow (WAWF). The WAWF website is located at https://wawf.eb.mil. There is no charge to use WAWF. Contractors must register to use WAWF at this website and ensure an electronic business point of contact (POC) is designated in the Central Contractor Registration site at http://www.ccr.gov within ten (10) calendar days after award of this contract. The Government will award this contract without discussions from Offerors. Basis for Award: Lowest Price Technically Acceptable (LPTA). "LPTA" means the expected outcome of the acquisition that, results from selection of the technically acceptable quotation with the lowest evaluated price. Pursuant to FAR 52.212-2, the criteria for evaluation are (1) Technical Specifications/technical support, (2) Price (to include shipping). EVALUATION FACTORS FOR AWARD : will be based on technical capabilities, price and price-related factors. FAILURE TO PROVIDE THE INFORMATION RELATIVE TO EACH EVALUATION FACTOR MAY RENDER YOUR OFFER NON-RESPONSIVE. a) Technical Capability will be based on technical features, and service literature. Descriptive literature or other documentation, and show how the offered service meets or exceeds the requirements as specified in the Performance Work Statement. b) Price includes all equipment, accessories, discount terms, and transportation. Award for All or None : The Government intends to award one (1) purchase order as a result of this solicitation. Offers received for less than the stated number if items listed in the solicitation will be considered ineligible for award. ###
