Description

The Great Lakes Acquisition Center (GLAC) is soliciting quotes for dialysis and plasmapheresis services for the Captain James A. Lovell Federal Health Care Center (FHCC), located at 3001 Green Bay Road, North Chicago, IL, 60064. Contractor shall provide technical support, personnel, equipment and supplies to perform hemodialysis, peritoneal dialysis, and plasmapheresis treatment services as necessary. The contract period of performance will be from January 1, 2016 through December 31, 2016, with four (4), one-year option renewal periods to be exercised at the Government's discretion. Contract type is firm-fixed (unit) price, indefinite quantity, indefinite delivery contract. All qualified offerors who believe they can satisfy this requirement are invited and encouraged to provide a quote. Offers, including options, will be evaluated for fair and reasonable pricing, technical capability to perform services, satisfactory past performance record, and socio-economic status (SDVOSB or VOSB). Large businesses must also submit an acceptable sub-contracting plan in accordance with FAR 19.704. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. The small business size standard for North American Industry Classification System (NAICS) code 621492 is $38.5 Million. Offerors are reminded to complete all sections of the solicitation, including 52.212-3 Offeror Representations and Certifications-Commercial Items and also through the System for Award Management (SAM) on-line at www.sam.gov. Electronic version of solicitation will be available on or about September 8, 2015. The date for receipt of quotes is September 28, 2015 at 1:00 P.M. CT. Electronic solicitation may be obtained FREE at the Federal Business Opportunities website, www.fedbizopps.gov, select "Find Business Opportunities" and then enter the solicitation number. Any amendments to this solicitation will also be available at this address. Offerors are advised that they are responsible for obtaining amendments.
