This is a combined synopsis/solicitation for a commercial supply and/or service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. This is a Request for Quote (RFQ) and the solicitation number is VA248-15-Q-2026. This is a 100% Service Disabled Veteran Owned Small Business (SDVOSB) Set Aside. The C.W. Bill Young, VAHC Contracting Office, Bay Pines, Florida, requires the following: DESCRIPTION OF REQUIREMENT: Hazardous and Non-Hazardous Waste Management and Disposal Services ITEMQTY UNITUNIT COSTTOTAL ANNUAL COST CLIN 0001 12 MO$__________$________________ The contractor is required to provide the necessary labor, supplies, and supervision for Hazardous and Non-Hazardous Waste Management and Disposal Services that include but are not limited to the following: collection of waste from the Satellites Accumulation Areas (SAA) within the facilities, proper identification and storage of hazardous waste at the Main Accumulation Area (MAA); proper segregation, packaging, document generation, labeling, transportation, recycling and final disposal of hazardous, universal (including Universal Pharmaceutical Waste), or otherwise non-hazardous waste currently accumulated at BPVAHCS in accordance with (IAW) the Statement of Work (SOW). Additionally, the contractor shall provide pharmaceutical formulary review; collection containers; technical consulting; electronic recordkeeping; emergency spill response; and staff training. PERIOD OF PERFORMANCE (POP): 1 October 2015 - 30 September 2016 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80 effective 02 Mar 2015. The North American Industry Classification System code (NAICS) is 562112 Hazardous Waste Collection. The Standard Industrial Classification (SIC) is S205 Trash/Garbage Collection Services. A firm fixed price contract will be awarded. All interested parties must bid on all items. Award will be made to the quotation which offers the Lowest Price Technically Acceptable. The Government also reserves the right to make no award at all. DELIVERY ADDRESS: This Waste Management is procured on behalf of the C.W. Bill Young VAHCS, 10000 Bay Pines Blvd, Bay Pines, FL 33744. The specific locations are provided in the attached Performance Work Statement (PWS). SITE VISIT: The site visit will be held this Thursday, 9/10/2015, at 10:00AM Eastern at the Bay Pines VA Medical Facility, 10,000 Bay Pines Blvd, Bldg. 100, Bay Pines, FL 33744. STATEMENT OF WORK HAZARDOUS AND NON-HAZARDOUS WASTE MANAGEMENT AND DISPOSAL SERVICE CONTRACT A.INTRODUCTION: The Bay Pines Veteran Healthcare System (BPVAHS), in accordance with Federal and State Environmental Regulations, requires a service contract for the management and disposal of Hazardous and Non-Hazardous Waste. B.SCOPE: A service contract is required to manage the hazardous and non-hazardous waste in the Bay Pines Veteran Healthcare System (BPVAHS) and its associate facilities located throughout the southwestern region of Florida. The VACHS facilities include, but are not limited to: C.W. Bill Young VA Medical Center, Out Patient and Rural Clinics, Community Based Outpatient. Below is the list of current facilities: 1.C.W. Bill Young VA Medical Center 10000 bay Pines Blvd Bay Pines, FL 33744 2. Bradenton Community-Based Outpatient Clinic 5520 S.R. 64 Suite 101 Bradenton, FL 34208 3. The contractor is required to provide the necessary labor, supplies, and supervision for the Waste Management Services that include but are not limited to the following: collection of waste from the Satellites Accumulation Areas (SAA) within the facilities, proper identification and storage of hazardous waste at the Main Accumulation Area (MAA); proper segregation, packaging, document generation, labeling, transportation, recycling and final disposal of hazardous, universal (including Universal Pharmaceutical Waste), or otherwise nonhazardous waste currently accumulated at BPVAHCS. Additionally, contractor will provide all necessary material and supplies for the proper management and disposal of waste. The BPVAHCS comprise of all three (3) generator status classification under 40 CFR 260-370; from Conditionally Exempt to Large Quantity Generator. Additionally includes Small Quantity Handler of Universal Waste and of Universal Pharmaceutical Waste. The contractor shall abide and provide appropriate services per the requirements applicable to each generator classification. C.PERFORMANCE PERIOD: The period of performance for this service contract is one (1) year from the date of issuance. Mainly, this contract will comply with the following regulations: a.Code of Federal Regulations 40 CFR 260-370 - Environmental Protection Agency b.Code of Federal Regulations 49 CFR Subchapter C Parts 105 -178 Department of Transportation c.FL Rule 62-730 d.Code of Federal Regulations 29 CFR 1910.38 - 1910.450 Occupational Safety and Health Administration (OSHA) F.GENERAL REQUIREMENTS: A.BPVAHCS guidance and procedures state that all Hazardous Wastes must be kept in an accumulation point, also known as a Satellite Accumulation Area (SAA), until a contractor is contacted for pick-up. The contractor must properly identify each waste, which may include the need to sample and analyse any or all generated waste for waste characterization and determination. Additionally, the contractor is required to properly segregate, identify, and store hazardous wastes in accumulation areas, as well as compliance with Federal, State and local requirements for accumulation areas. B.The contractor's response to this solicitation must include the identification of each waste, a description its final disposal method (either recycled/treated/land filled), and a demonstration of the cost-effectiveness of each type of disposal for each specific waste. The contractor must provide specific pricing information, for the disposal of each waste delineated in this contract. This includes different unit sizes for each waste. An example of the pricing information required is illustrated in Table 1. C.The contractor must provide pricing information on specific gases cylinder management and disposal according to size and type of gas. The contractor must also provide pricing information for the mobilization and treatment of high hazard chemicals. PRIMARY DISPOSAL METHOD: WASTEWATER TREATMENT55 DF$ 16$ 5$ D.This contract shall cover all RCRA hazardous waste and non-hazardous waste as determined by BPVAHCS. Normally, this includes waste generated by typical healthcare facility operations. Although a specific waste disposal pricing list will be generated by the Contractor, additional wastes may be required to be included at a later time, as needed by BPVAHCS. An example of such waste streams may be the result of a new procedure, management practices, new equipment installed, expired chemicals, among others, at the BPVAHCS facilities. Therefore, this contract shall encompass the management and disposal of a comprehensive spectrum of waste streams, as required by BPVAHCS. It is anticipated that the waste streams may vary and change overtime. This contract shall accommodate for these changes. E.This contract shall cover all Hazardous Waste Codes (per RCRA). F.The contractor is responsible for the proper management, labelling, packaging, and collection and transportation from point of generation, to accumulation sites, to ultimate disposal. The contractor shall ensure waste is transported to a properly permitted Treatment, Storage, and Disposal Facility (TSDF) or recycling facility which is owned and operated by the same contractor. G.The contractor shall generate, provide and maintain all required documentation, including, but not limited to: manifests, land disposal restriction forms, packing lists, and waste profiles. The contractor must ensure that all hazardous and non-hazardous wastes are disposed of properly to avoid the possibility of a fine or penalty by Federal and State environmental agencies. The contractor shall be held responsible for any fines/penalties assessed due to state, local or federal laws being violated by the contractor. H.All documentation and reporting requirements for hazardous and toxic waste disposal is to be accomplished by the contractor, with copies provided to BPVAHCS. The contractor is to be held responsible for the waste from the accumulation point until its ultimate disposal, including its proper handling, identification, transportation, packaging, labelling and paperwork required. I.All work performed by the contractor shall comply with all applicable Federal, State and local laws and regulations, including, but not limited to, those promulgated under the Resource Conservation and Recovery Act, the Toxic Substances Control Act, the Occupational Safety and Health Act, the Hazardous Materials Transportation Act, and applicable Department of Transportation regulations. J.The Bay Pines VA Healthcare System shall properly place all hazardous and toxic material for disposal under task order against this Indefinite Delivery, Indefinite Quantity (IDIQ) contract. The task order will be accompanied by documentation and markings to properly identify the waste if available. The exception may be in the case of wastes generated from broken or leaking containers or unidentified containers where it may be necessary to determine whether such wastes and cleanup materials exhibit the characteristics of hazardous wastes as defined by Federal or State regulations. In these instances, the contractor shall provide for any additional testing and/or analysis required for characterization, transportation, treatment, recycling, or disposal of the hazardous or toxic waste. K.The contractor must perform all operations in a prudent, conscientious, safe and professional manner. At a minimum, contractor's personnel and equipment shall comply with applicable state, Federal, and local laws, safety regulations and procedures, and contractor will ensure its agents, employees, and subcontractors perform in a safe manner. The contractor shall ensure that all personnel involved in handling and packaging the hazardous waste be trained for the level of expertise required for the proper performance of the task and, in particular, in the areas of chemical incompatibility, general first aid procedures and spills. Handling and personnel protective equipment shall be provided by the contractor and must be appropriate to ensure safe handling of the hazardous waste. The contractor agrees that his personnel and equipment are subject to safety inspections by BPVAHCS personnel and state authorities while on BPVAHCS property. L.The contractor shall, without additional expense to the BPVAHCS, be responsible for obtaining any necessary licenses and permits, and for complying with any applicable Federal, State and local laws, codes, and regulations in connection with the execution of the work to be performed under this contract. This includes ensuring that the BPVAHCS hazardous or toxic wastes are sent to licensed, permitted, and approved disposal sites. M.BPVAHCS shall not furnish any equipment or personnel to assist the contractor in the performance of the contractor's responsibilities under this contract. The Contractor understands that any such offers of assistance are unauthorized, and the contractor shall not accept any such offers. N.The contractor agrees that, for those portions of the services provided on BPVAHCS property, the services will be provided during the normal hours of operations for the BPVAHCS facility. The normal hours of operations hours are Monday thru Friday from 7:30 AM through 5:00 PM. O.The awardee shall be a Service Disabled Veteran Owned Small Business (SDVOSB) and must provide 50% or more of the work for this contract. P.The contractor shall be equipped with and expected to respond to environmental emergencies on a 24/7/365 schedule. The emergency response service is expected to be on an "on call" basis and a response time will need to take place within 45 minutes of the initial communication. VI.SPECIFIC REQUIREMENTS: 1.Waste Identification, Characterization and Determination: a.Table 2 describes the hazardous and non-hazardous wastes that might be potentially generated at typical BPVAHCS facilities. Wastes shall be characterized per 40 CFR Part 260. c.The contractor shall prepare all waste analysis sheets identified to a specific contract line item that may be required for disposal and provide a copy to BPVAHCS. d.If the contractor must perform detailed analysis for disposal, copies of the results identified to a specific contract line item shall be provided to the BPVAHCS. Any detailed analysis must comply with all Federal, State, and local requirements. e.Analysis must include determination if hazardous waste is to be incinerated, treated, recycled, or ultimately disposed of in a landfill and a cost benefit analysis of such determination. Analyses will be provided to BPVAHCS. The method of ultimate disposal will be determined by the Chief, Safety and Emergency Management Service or its designee of BPVAHCS. 2.Labeling and Marking: a.Each container shall be labeled and marked by the contractor accordance with 40 CFR Part 260 and 49 CFR Subchapter C Parts 105 -178. 3.Waste Collection from Satellite Accumulation: a.The Contractor is responsible for the collection of all hazardous and non-hazardous wastes from all of the designated Satellite Accumulation Areas (SAA) within BPVAHCS, as required. The contractor shall ensure that the SAA and associated waste containers are in compliance with applicable RCRA regulations. b.The contractor shall perform weekly SAA inspections utilizing BPVAHCS inspection form. c.All items collected on this contract must be properly segregated and kept physically separated from any other items in accordance with chemical compatibility guidelines until the initial treatment, storage disposal facility (TSDF) is reached. d.The contractor must ensure that all waste containers are appropriately marked. In addition, the contractor must ensure that there is a clear audit trail for all items until final treatment/disposal is accomplished. This will ensure that all wastes are tracked from cradle-to-grave. 4.Waste Transportation to Main Accumulation Area: a.The Contractor is responsible for transporting all collected hazardous and non-hazardous wastes from all of the Satellite Accumulation Areas (SAA) to the Main Accumulation Area. b.The Contractor shall conduct sound handling practices and procedures while transporting the hazardous and non-hazardous wastes to avoid any spill incidents and exposures. c.Special attention must be lent to the transportation at the C.W. Bill Young VA Medical Center's chemical transport. Due to the nature of the campus, vehicular transport of chemicals will often be necessary. While this task is performed, secondary containment for all transported containers shall be used. d.All items stored at the Main Accumulation Area (MAA) must be properly segregated and kept physically separate from any other items in accordance with chemical compatibility guidelines until the initial treatment, storage disposal facility (TSDF) is reached. e.The MAA must be kept clean, organized, and secured at all times. f.Any deficiencies in accordance with RCRA requirements for MAA (90/180-day storage area) must be immediately corrected and notified to BPVAHCS. 5.Transportation to TSDF: a.The contractor will comply with, and ensure that all applicable subcontractors comply with, all requirements of United States Department of Transportation (DOT) regulations, 49CFR Parts 100-199, regarding waste transportation under this contract. Complete compliance with these regulations shall include, but not be limited to, ensuring that emergency response information is carried on transport vehicles and maintained at facilities where hazardous materials are received, stored, or handled during transportation. Shipping papers will contain an emergency response telephone number supplied by the disposal contractor which is monitored 24 hours a day in order to provide immediate, detailed emergency information to personnel reacting to emergencies. Technical names will be included in the descriptions of materials reflected on shipping papers by "not otherwise specified" (n.o.s.) descriptions. Definitions of the terms used in this clause are those used in 49 CFR 100-199. Contractors must ensure that off-site shipments of hazardous waste are labeled, marked, and placarded according to the U.S. DOT requirements. Contractor remains responsible for proper management of waste even after it leaves BPVAHCS Facilities. This includes responsibility for any spill while in transit. b.The contractor shall fill in Table 3, indicating each transporter that may be used to transport hazardous or toxic property under this contract. Each transporter must be properly licensed, trained, permitted, conform to the requirements of this contract and be in good standing with the regulatory community. The contractor agrees that no transporters other than those listed below will be used without obtaining the prior written approval from BPVAHCS. The applicable state hauling permit numbers must be provided for the state(s) where the pick-up sites are located, the state(s) where the interim storage facilities are located as listed in their TSDF plan and the state(s) where the disposal facilities are located as listed in the TSDF plan. If more space is needed, append an attachment to the proposal indicating that it is a continuation. d.The transporters approved for use under this contract are only approved subject to all services being performed in accordance with all Federal, State, and local laws and regulations and the transporters permit. e.BPVAHCS does not warrant that the drums or containers are suitable for transportation in accordance with DOT regulations. The contractor is cautioned to ascertain and assess the need for over-packing or re-containerizing, as required. f.The contractor is responsible for loading, including furnishing all the equipment necessary for loading. Unless otherwise provided in the contract, loading will not be performed on Saturdays, Sundays, holidays, or any day that the facility where the wastes are located is closed. i.In case of emergent situations or in preparation to a weather event, waste pick up might need to be arranged on weekends, holidays or after normal ours. This coordination will be requested by and arranged through BPVAHCS pertinent personnel. g.Bulk Waste Items will be measured by one of the following methods. The method used will be whichever is most accurate and agreed upon by the contractor and BPVAHCS representative. i.Actual weight using BPVAHCS or other commercial scales. ii.Calculated weight. As an alternative to actual weighing, the weight of bulk shipments may be computed, provided that the specific gravity of the material is known and the volume actually picked up is determined. For example, 2,000 gallons of liquid with a specific gravity of 1.4 = calculated weight of 23,344 pounds (2,000 x 1.4 x 8.337, where one gallon of water weighs 8.337 pounds). Specific gravity will be obtained from a waste profile sheet. iii.If necessary to pump liquids, solids, and sludges from tanks, the contractor shall furnish a collection vehicle equipped with vacuum pumps and hoses. Each pumping truck shall have at least 100 linear feet of hose. Pumps and hoses shall have the capability to safely handle the types of wastes to be collected and be able to remove all liquids, solids, and sludges from tanks that can be removed without agitation or introduction of other materials into the tanks. h.For Non-Bulk Waste Items, the contractor shall provide portable scales for outweighing of property. Portable scales must have a minimum capacity for 1,500 pounds. For the purpose of this contract, scales permanently affixed or built into a vehicle are considered portable scales. Prior to the use of portable scales at each pickup site, the contractor must demonstrate reasonable weight accuracy to the BPVAHCS representative. Only materials to be removed by the contractor will be weighted. Pallets, boxes, stripping, etc., which are not integral parts of the packaging and are not being removed by the contractor shall not be included in the weight. i.Empty Containers - Prior to reuse or sale of empty containers, the contractor shall: i.Comply with the cleaning requirements of 40 CFR 261.7. ii.Obliterate all markings and labels. j.The contractor shall be exempt from this requirement only if the containers are crushed and sent to a scrap operation, crushed and sent to a landfill, or the containers are physically disposed of concurrently with their contents (i.e., incineration/landfill). k.The contractor shall package, mark, label and placard all items in such a manner that all applicable Federal (EPA & DOT), State, and local regulations are met. l.Packaging, shipping names, marking, labelling placarding, etc., under the terms of this contract will be in accordance with 49 CFR and 40 CFR. m.If items must be packaged for proper shipment, the contractor shall perform such repackaging and furnish all required materials. When repackaging is necessary, the contractor shall be responsible for disposal of the original container and placarding in a manner that complies with all applicable regulations. n.The contractor shall also provide and affix appropriate placards to each vehicle prior to leaving BPVAHCS premises and ensure that all other DOT transporters requirements are met. o.If the contractor elects to package compatible items in the same container, then the contractor must provide an all-inclusive packing list showing each item and its respective quantity. This list shall be placed outside the outer most containers. A copy of the packing list must be attached to the manifest. p.Contractor furnished over-pack containers and materials will not be included in the total weight calculations. 6.Treatment and Final Disposal: a.For purposes of this contract, final treatment/disposal means either: treatment so that such wastes no longer meet the definition of a hazardous waste as defined in 40 CFR 261; or treatment of a waste by a RCRA handling method specified in 40 CFR Parts 264/265 appendix 1, Table 2, paragraph 2. b.Waste handling costs that describe methods of storage do not meet the definition of final treatment nor final disposal under this contract. c.Interim treatment of the waste such that the waste still meets the definition of a hazardous waste as defined in 40 CFR 261 does not meet the definition of final treatment under this contract. d.Final disposal means disposal of a waste by a RCRA handling method specified in 40 CFR, parts 264/265, appendix 1, table 2, paragraph 3. All facilities used for interim treatment, final treatment or final disposal of items on this contract shall have as a minimum, an EPA/State approved interim status permit showing EPA hazardous waste numbers described in 40 CFR 261, subparts c and d, for each waste the facility is permitted to handle. e.Mere acceptance of the hazardous waste at a properly permitted treatment, storage, or disposal facility (TSDF) does not meet the definition of final treatment nor final disposal under this contract. It is the contractor's responsibility to obtain all necessary documentation to prove that the final treatment or final disposal of all items (as defined in paragraph 2.2.9.1 above) has been accomplished. This documentation shall be attached to the certificate of disposal and submitted with, or prior to, the invoices. f.The contractor will provide all services necessary for the final treatment/disposal of the hazardous wastes listed in the price schedule in accordance with all local, state, and federal laws, and regulations, and the terms and conditions of this contract. These services shall include all necessary personnel, labor, transportation, packaging, equipment and the compilation and submission of all documents required. g.The contractor shall manage and dispose the manifested waste for incineration. Unless otherwise identified in the profile all waste shall be treated in the following order. i.Incineration ii.Fuel-blending iii.Recycling iv.Solidification / Landfill h.BPVAHCS will notify the contractor when a waste pickup and shipment is required from the Main Accumulation to the TSDF. A date will be scheduled with both parties. i.The Contractor shall identify each treatment, storage and disposal facility (TSDF) which may perform treatment, storage or disposal services under this contract. Identified TSDF's will need to be owned by or in contractual agreement with the contractor. j.Each facility must have, as a minimum, EPA RCRA interim status or state approval as a treatment or disposal facility and be in good standing with the regulatory community. k.The contractor agrees that no facility other than those initially approved for use under this contract will be used, without first obtaining the written approval of the BPVAHCS. l.The appropriate permit/license number, name and address of the permitting/licensing agency shall be provided. m.The TSDFs approved for use under this contract are only approved subject to all services being performed in accordance with all Federal, State and local laws and regulations and the TSDF's permit. n.The contractor must have the capability to meet the removal and disposal timeframes required by Federal/State regulations and BPVAHCS schedule requirements for all waste streams. o.At least one interim storage facility for each waste stream shall be included in the proposal. p.Treatment of hazardous waste (including solidification) on BPVAHCS property is not permitted. Treatment is defined as any process which meets the definition of treatment as set forth in applicable Federal, State and local laws and regulations. 7.Documentation Requirements: a.A uniform hazardous waste manifest is required for the removal of waste from BPVAHCS property. The contractor shall obtain, generate, review and provide manifests for all of the wastes generated at BPVAHCS. b.The contractor shall provide the manifests to BPVAHCS for review of completeness and accuracy prior to removal of any wastes from BPVAHCS property. c.The contractor shall provide BPVAHCS with a copy of the manifest(s), land disposal restriction notification(s), and any other shipping documents for review by the appropriate BPVAHCS official at least 2 hours prior to removal, whenever possible. d.The pickup manifests will be reviewed and signed by the appropriate authorized BPVAHCS official. A list of BPVAHCS approved signatories will be provided for reference purposes. Completed copies of all manifests shall be furnished to BPVAHCS and arrangements must be made for the TSDF signed copy is mailed back to the Office of Safety & Emergency Management Service, as well as submitted as attachments to all invoices. e.Manifests shall be submitted in such a manner as to comply with the timeliness prescribed by the State and EPA regulations. f.The contractor is required to provide BPVAHCS with two (2) copies of any manifest which documents the movement of waste after it has departed BPVAHCS property within 30 calendar days after acceptance for transportation. For example, if waste is shipped from the accumulation site to a facility for storage or treatment which does not meet the definition of final disposal, then a manifest detailing the acceptance of the waste into the additional facility must be provided within the prescribed timeframe. g.Each manifest, as well as other documentation required herein, shall be clearly and distinctly marked with the contract number and delivery order number as applicable. If blocks are not provided, the contract and delivery order information shall be placed in the upper right hand corner of each document. h.BPVAHCS Waste Profiles shall be maintained by the Contractor and a copy will be left kept at BPVAHCS. These waste profiles shall be accessible and readily available. 8.Training: a.All Contractor personnel working at BPVAHCS premises under this contract shall have the required RCRA trainings per 40 CFR 260-370 and 29 CFR 1910.120. A copy of these training certificates shall be furnished to BPVAHCS Safety & Emergency Management Service. 9.Spill Response Operations: a.The contractor is solely responsible for any and all spills or leaks and subsequent clean ups that may be required during the performance of this contract which occur as a result of, or contributed to, by the actions of its agents, employees, or subcontractors. The contractor agrees to clean up such spills or leaks to the satisfaction of the BPVAHCS and in a manner that complies with applicable Federal, State, and local laws and regulations. The clean up shall be at no cost to BPVAHCS. b.The contractor shall report all such spills or leaks, regardless of their quantity, to BPVAHCS Safety & Emergency Management Service immediately upon discovery. A written follow-up report shall be submitted to the BPVAHCS no later than 24 hours after the initial telephone report. The written report to BPVAHCS shall be in narrative form and as a minimum include the following: i.Description of the item spilled (including identity, quantity, manifest number, etc). ii.Whether amount spilled is EPA/State reportable. iii.Exact time and location of spill, including a description of the area involved and digital picture documentation of the spill and the clean up. iv.Containment procedures initiated. v.Summary of any communications contractor has with press, State officials, or Police Service officials. vi.Description of clean-up procedures employed or to be employed at the site including final disposition and disposal location of spill residue. c.The Contractor shall also provide Emergency Spill Response Services, as required, for any incidents in which a hazardous material or waste spill should occur. i.The Contractor shall provide Emergency Response service with an Incident Command Certified person on site within 45 minutes or less of the original call. This action must be followed by equipment mobilization and initiation of emergency control and clean up with in 2hrs. ii.This service is of an extraordinary circumstance which would require the pumping of liquids, solids, and sludge from tanks. The contractor shall furnish collection vehicles equipped with vacuum pumps and hoses. Each pumping truck shall have at least 100 linear feet of hose. Pumps and hoses shall have the capability to safely handle the types of wastes to be collected and be able to remove all liquids, solids, and sludges from tanks that can be removed without agitation or introduction of other materials into the tanks. This service will be 12 of waste management supplies at BPVAHCS premises, which include, but not limited to: iii.Drums (assorted capacity, material and lids) iv.Vermiculite v.Spill Pads vi.Over-packs vii.Buckets (batteries, pails, etc) viii.Fluorescent Bulb Containers (DOT approved) ix.Cubic Yards (Flex Bins) x.DOT Labels xi.Hazardous, Non Hazardous, Universal waste and Universal Pharmaceutical waste shipping labels d.All material and supply purchases shall have prior approval by BPVAHCS. The Contractor is responsible for notifying BPVAHCS when supply and material stock levels require replenishment. 10.Reporting: a.A pickup report shall be completed by the contractor at the time of each pickup and shall accompany each shipment. Completed copies are to be furnished to the Safety and Emergency Management Service of BPVAHCS. A list of all of waste to be sent for disposal shall be listed on the appropriate forms and the contract number must be referenced. b.The contractor shall prepare a Certificate of Disposal. This certificate is to be provided prior to submittal of the invoices. Any discrepancies between the BPVAHCS documented waste inventory and the actual shipment for disposal, must be thoroughly described, documented and authorized prior to completion of the shipment. c.The contractor shall prepare a Certificate of Recycling for any waste that is recycled. This certificate shall be provided prior to the submittal of invoices. A copy of each certification of recycling, signed by a responsible company official, will be provided to the Safety and Emergency Management Service of BPVAHCS. d.The contractor shall prepare quarterly stewardship reports detailing all hazardous and non-hazardous waste disposals, recycling data and spill response activities. The report shall provide detailed statistics for each of the 9 work sites within the Healthcare System. The contractor shall present the quarterly report during the quarterly stewardship meeting that will be conducted at the CW Bill Young Medical Center. e.The contractor shall prepare an Annual Report describing all hazardous and non-hazardous waste disposals and recycling data. The Annual Report shall be submitted to BPVAHCS no later than January 10th. 11.On-site Services: a.The Contractor shall perform the following tasks under the On-site Services: i.Collection of waste from all of the generation points. ii.Weekly inspection of the Satellite Accumulation Areas. iii.Maintenance of the Main Accumulation Areas (90/180-days). iv.Waste characterization, when deemed necessary v.Lab packing vi.Material and supplies inventory vii.Chemical consolidation with appropriate Personal Protective Equipment. Consolidation of waste will need to be approved by the Chief of Safety and Emergency Management Service or its designee. viii.Transfer of waste from the SAAs to the MAAs. b.BPVAHCS reserves the right to request and/or determine the type of on-site service personnel which is determined to be best suited for the needs of the VACHS facilities. 12.Rental: a.The Contractor shall provide, as required by BPVAHCS, rental services for Roll-offs and other types of waste containers. b.Pricing information for each available rental services must be provided.
