- THIS IS A SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified small and large business sources; (2) if small business, whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work require under the proposed contract. The NAICS code is 562112 Place of Performance: 1)W.G Hefner VA Medical Center, 1601 Brenner Ave., Salisbury, NC 28144 2)Charlotte, CBOC, 8601 University East Drive, Charlotte, NC, 28213 3)Winston Salem OPC, 190 Kimel Park Drive. Winston Salem, NC, 27103 Industry Survey Responses Requested: Sources Sought which can provide/perform medical waste pickup services: Pickups shall be performed by contractor during normal administrative hours of 8:00am to 4:30 pm Monday through Friday. Exceptions will be a federal holiday and any time change that is mutually agreed upon by the contractor and Contract COR. Notify the appropriate facility contact upon arrival and provide for a signed verification of a service ticket or other document and or manifest at time of service. This is required to verify proof of service before payment can be authorized The waste will be removed from the premises on week days, between the hours of 8:00 AM to 4:30 PM. In the event a holiday falls on one of those days then waste will be removed the preceding day All waste shall be weighed at site an entered on a Manifest that guarantees tracking of the waste from the premises through the destruction by the Incinerator or Microwave. After the waste is destroyed a copy of the manifest certificate will be sent to the VA (Within 30 days from disposal) as proof of the destruction. Written back-up plan as to how the company will handle the waste disposal in the event their system fails. The alternate incinerator company's name, permit and license will also be submitted to the VA. A copy of all state and federal licenses and permits on the incinerator and operators, and a statement on the contractor's letterhead that the incinerator meets all state and federal codes and restrictions. A letter stating the contractor assumes all liability related to the waste disposal from the time the boxes of waste are removed and until the waste is destroyed. Invoices will be paid on a monthly basis and the VA will pay the scale weight at the incinerator or microwave. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements by September 11, 2015 no later than 8:00am (EST) via email to Sarita.foxworth2@va.gov. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER SEPTEMBER 11, 2015. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. (5) No Proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s.)
