SOURCES SOUGHT This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NACIS 562211. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. Michael E. DeBakey VA Medical Center 2002 Holcombe BLVD Houston, TX 77030 (Draft) Statement of Work SCOPE. The Michael E. DeBakey VA Medical Center (MEDVAMC), 2002 Holcombe Blvd., Houston, TX 77030-4298 is seeking a Contractor to provide medical/special, pathological and residual chemotherapy waste treatment and disposal service on a fee-per-pound basis, plus a separate per-drum-cost disposal contingency for all services related to disposal of DOT Infectious Substances (i.e., Category A and Category B) to the MEDVAMC Houston, and a total of nine (9) each Community Based Outpatient Clinics (CBOCs) (i.e., Beaumont, Lufkin, Galveston Island, Richmond-Rosenberg, Texas City, Conroe, Katy, Tom Ball, and Lake Jackson, Texas) with the specific services requirements identified in this SOW. The specific services as a minimum shall include all materials, manpower, process equipment, licenses, permits, fees, fuel surcharge, transportation and computerized documentation for the complete treatment and disposal of an estimated total of 86,000 lbs./monthly of medical/special, pathological and residual chemotherapy waste in an Approved or Alternate Treatment Methods Technology for Special Waste from Health Care-Related Facilities in strict compliance with all applicable Federal (CDC, OSHA, DOT, EPA), State (TCEQ, TDH), and Local laws, regulations, ordinances, and codes, etc. ? All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, business size, DUNS #, and GSA contract number (if applicable) pursuant to the NAICS code 562211. Responses to this notice shall be e-mailed to Darnell.Rias@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than Thursday, September 10, 2015 at 2p.m. CST. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
