Description

This is a combined synopsis/solicitation for commercial acquisition prepared in accordance FAR parts 12 and 15, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number is VA248-15-R-1760 and is issued as a Request for Proposal (RFP). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-76, effective 25 August 2014. This solicitation starts 09/04/2015 and will end 09/18/2015 at 2:00 pm EST. Network Contracting Office (NCO) 8 intends to release Solicitation VA248-15-R-1760 for Durable Medical Equipment (DME) on behalf of the Miami VA Healthcare System. This solicitation will require contractor to store and deliver assigned durable medical equipment to include supplies, materials, equipment, transportation of equipment, equipment services, labor, supervision, patient education, safety management, infection control, as necessary for veterans. Occasionally, the contractor may be required to store excess equipment through adequate warehouse space as per the requested amount of the supported facility. The purpose of this contract will be to provide DME services 24 hours per day, 365 days a year. Miami's area of responsibility covers all Department of Veterans Affairs Beneficiaries residences in designated counties. The intent for this contract is to meet or exceed the requirements for a one year contract period. Please follow all instructions given in attached Solicitation #VA248-15-R-1760. Award from this solicitation will be set-aside for Veteran Owned Small Business Contractors. The North American Industry Classification System (NAICS) for this procurement is 621610, Home Health Care Services; size standard in number of employees is 500. This will be a Firm Fixed Price, Indefinite Delivery Indefinite Quantity Contract for a period of one (1) year. The government anticipates awarding a single award. All qualified veteran owned small business sources are encouraged to submit a proposal. Questions should be directed to the Contract Officer at the email address listed below. All offerors must be registered in SAMS data base, see https://www.sam.gov and complete the online Reps and Certs. Amendments to the solicitation will be posted at this website only. All offerors must obtain amendments at this website; hard copies will not be mailed. All questions must be submitted in writing by email no later than 09/11/2015 @ 3PM EST.
