- This is a Sources Sought (SS) is to obtain information regarding the availability and capability of qualified small business concerns; and their size classification relative to the North American Industry Classification System (NAICS) code 493110 with a size standard of $27.5M. The responses to this announcement will assist in determining the availability of small businesses in consideration of a socio-economic set-aside. The requirement is to provide all necessary labor, equipment, services and supervision required to render a complete Warehouse Manage System (WMS). Responsible for the storage, management and delivery of Medical/Surgical supplies in support of the VA Pandemic Influenza Plan for the following Veteran Integrated Service Network (VISN) 8 Healthcare facilities: James A. Haley VA Healthcare System, Bay Pines VA Healthcare System, Orlando VA Medical Center, and North Florida/South Georgia VA Healthcare System. All supplies must be stored in a clean, uncluttered, and temperature controlled environment. See Draft Statement of Work (SOW) for additional information. The Government may use the responses to this SS for information and planning purposes. The Draft SOW describes only the currently contemplated possible scope of services and may vary from the work scope in a final SOW included in any Request for Proposals (RFP) or Request for Quotes (RFQ). A single firm-fixed price contract is contemplated with the anticipated period of performance of: 1 October 2015 thru 30 September 2016 with possible four one-year option periods. Interested parties submitting a response to this inquiry must include the following information: 1. Name and address of company 2. Primary point of contact, including phone number and e-mail address 3. Applicable socioeconomic categories and any pertinent information, which demonstrates firm's ability to meet the requirement 4. STATEMENT OF WORK Warehouse Management System (For Pandemic Cache) 1. GENERAL REQUIREMENTS 1.1 Contractor shall furnish all necessary labor, equipment, services and supervision required to render a complete Warehouse Manage System (WMS). Responsible for the pick-up, storage and distribution of Medical/Surgical supplies in support of Veteran Integrated Service Network (VISN) 8 Healthcare facilities: James A. Haley VA Healthcare System, Bay Pines VA Healthcare System, Orlando VA Medical Center, and North Florida/South Georgia VA Healthcare System. All supplies must be stored in a clean, uncluttered, and temperature controlled environment. 1.2 Contractor shall be responsible for all management and supervision of the contract work. Contractor and/or on-site supervisor shall be available at all times when the contract work is in progress, to receive notices, reports or requests from the Contracting Officer's Representative (COR). Contractor shall provide in writing to the Contracting Officer (CO) prior to the contract starting date, the name, telephone number and address of onsite supervision and/or project management. 1.3 Contractor shall provide complete WMS services for an average of 290 pallets, with expansion capabilities up to a maximum of 700 pallets, of Medical/Surgical Supplies, consisting of 30 lines (estimate of Medical/Surgical Supplies by commodity). Product line may change based on the protocol set by the Center for Disease Control and Prevention (CDC) and/or product availability. ITEMSEST. SCOPE OF WORK 2.1 The warehouse facility shall be owned or leased by the contractor, and shall be located within a 15 mile radius of the James A. Haley Veterans' Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612. 2.2 Contractor shall be responsible for relocating supplies from the current location in the event that a new contract is awarded to other than the incumbent. The address for the current location is 5210 Eagle Trail Dr. Tampa, FL 33634. 2.3 Contractor shall comply with all applicable Federal, State, local and industry safety, i.e., OSHA, Department of Labor (DOL), and health standards, regulations and statutes pertaining to the operation of the warehouse and/or distribution of commodities identified or associated with this Statement of Work. 2.4 Contractor's facility shall meet all industry standards for fire alarm and suppression system for the commodities being warehoused. Facility shall also have individual 24/7 monitored alarm access. 2.5 VA Medical/Surgical Supplies shall be segregated by a cage or other defining material for quick identification purposes. Supplies shall be stored at a temperature controlled environment not to exceed 78 (degrees) Fahrenheit and a humidity range of 20% to 60%. 2.6 Contractor shall maintain an electronic database of all merchandise stored and cross reference this data with VA IFCAP numbers. Information shall contain item description, stock number, case quantities on hand, packaging multiples, vendor information, quantity per pallet, and pallet count. 2.7 Contractor shall provide full access for VA personnel for warehouse inspection during normal business hours (8:00AM - 4:30PM, Monday - Friday). In addition, access via individual keyed entry to the supply/products shall be available 24/7. 2.8 Contractor shall provide delivery of Medical/Surgical Supplies from the warehouse facility to any of the medical facilities listed in this SOW. This delivery will not be in a recurrent base. Delivery will be needed in emergency and non-emergency cases, as needed and requested by the VA. During an emergency, supplies shall be delivered within 24 hour after notification, 7 days a week. Any other time, supplies shall be delivered within 24-48 hours after notification, Monday-Friday. If any of the stored Medical/Surgical Supplies are used, the VA will order the required supplies from the Medical/Surgical Prime Vendor to be delivered to the contractor's warehouse facility to replenish the used supplies. 2.9 Contractor shall provide the COR the following: "Monthly inspection reports of product inventory for expiration dates. Notify the COR of any product that expires within 12 months. "Monthly inspection reports of equipment used to move supplies (forklift, power jacks, etc.). "Quarterly order stock rotation by pulling orders from the oldest available stock for any order received and replacing it with fresh product from VA inventory. A minimum six months shelf-life is required. Stock items that are rotated must be highlighted with new expiration date.
