Description
Amendment 03 to this notice does not extend the due date for responses. The Question period is closed. Electronic submission is acceptable and preferred, but if submission is via hard copy, no more than two (paper) copies (one marked Original) and one on CD/DVD shall be submitted. Regardless of the format of submission, all Files must be compatible with Adobe 8.0 or higher and Windows 7. PWS Paragraph 1.3.6 is revised and paragraph 1.3.7 is added as shown in the solicitation attachment file titled, "Questions and Answers", uploaded on 03 Sept 2015. The Madigan Army Medical Center has a requirement for support of the Mobile Obstetric Emergencies Simulator and Obstetric Patient Safety Research Programs - Interval Training for Obstetrics Emergencies Research Program Manager. The contractor shall provide all labor, travel, administrative, and logistical support necessary to complete the research, development, and validation of specific simulation curriculum to improve provider preparedness for obstetric emergencies. The Obstetric Emergencies Research Program Manager shall evaluate current obstetric care and provide timely feedback on current practice standards. Contract support includes day-to-day study operations, recruitment efforts, assessments/questionnaire administration, training compliance, gathering/compiling data and providing outcome metrics. Increasing the provider's confidence and improving patient outcomes to include lower rates of neonatal injury and more rapid response by the perinatal team when an emergency cesarean section is required. The Obstetric Emergencies Research Program Manager will perform primary activity at Madigan Army Medical Center, Joint Base Lewis-McChord, Washington 98431. The valuable data gathered from the study will benefit patients by improving the training physicians receive in obstetrical emergencies directly improving patient outcomes by building better confidence when performing these procedures and will extend to the entire Army. This is a combined synopsis/solicitation issued as a Request For Quotation (RFQ), for commercial items prepared in accordance with Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Firm Fixed Price (FFP) proposals are being requested and a written solicitation will not be issued. Please include your DUNS Number and CAGE Code with your offer. Responses shall include the following: A Technical Proposal that provides documentation to reflect the knowledge and capabilities to meet the requirements of the Performance Work Statement (PWS), Quality Assurance and Surveillance Plan (QASP), Past Performance documentation, and a Price Proposal. The Price Proposal should include sufficient information to establish that the proposed price is fair and reasonable. See attached solicitation which includes evaluation criteria and (PWS). This requirement is a total 100 % small-business set aside. The applicable North American Industry Classification Standard (NAICS) Code is 541712. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation (FAR) and Defense Federal Regulation Supplement (DFARS) through the Federal Acquisition Circular 2005-83 effective 2 Jul 2015. All questions regarding this RFQ are due no later than 5:00 p.m. (EST) on Thursday, Aug 27, 2015 and shall be submitted electronically to Sandra O'Connell at sandra.j.oconnell.civ@mail.mil or Monesha Mitchell at monesha.r.mitchell.ctr@mail.mil. Answers will be posted via amendment.
