Project Summary: Adams and Associates, Inc, contracting for the U.S. Department of Labor for the Atterbury Job Corps Center-a 410 student residential training facility located at 3129 E. Edinburgh St., Edinburgh Indiana 46124 and IndyPendence Job Corps Center-a 100 student non-residential training facility located at 222 E. Ohio St., Indianapolis, Indiana, is currently soliciting written bids for an on-site Dental Assistant Services Provider. All quotes should be submitted in writing to the Purchasing Department by Monday, September 28th, 2015, 3:00 pm east coast time. Quotes received after the deadline and/or without all requested documentation will not be considered. Scope of Work ID# AT2015-112 must be clearly indicated on submitted bid. I. SCOPE OF WORK: The Dental Assistant agrees to provide students of the Atterbury Job Corps Center and IndyPendence Job Corps Center with the services stated herein. Such services will be provided as authorized by the Center Director or his/her representative. • Dental Assistant Services will be provided in the amount of 21 hours per week. Of the 21 hours per week, 17 hours shall be provided at the Wellness Center located at Atterbury Job Corps Center in Edinburgh Indiana and 4 hours per week will be provided on-site at the facilities located at the IndyPendence Center in Indianapolis, Indiana. • Period of Performance is from 12/1/15-11/30/15. • Proposal should include cost price per hour. Services to be provided will include, but will not be limited to those oral health and wellness services specified in the Job Corps Federal Regulations (Title 20, Code of Federal Regulations 638.510; the Job Corps Policy and Requirements Handbook, Chapter 6, Section 6.10, R2; and applicable Technical Assistance Guides). The Dental Assistant will assist the Dentist in overseeing the development, implementation, and monitoring of the total Center oral health and wellness program, which is subject to the approval of the Center Director. The Center Dental Assistant shall assist the Dentist with performing dental services for students, including but not limited to: A. Assessment and diagnosis, to include: • Dental readiness inspection within 14 days after entry by the Center Dentist or designee (i.e., dental hygienist or dental assistant) to identify urgent care need for oral conditions that if not treated are expected to result in dental emergencies in the near future. • Elective oral examination upon student request, including x-rays to precede dental treatment. B. Treatment, to include: • Dental procedures to treat oral disease and correct oral health conditions that may represent employability barriers. Specific procedures include: 1. Restorations 2. Extraction of pathological teeth 3. Root canal therapy on anterior and other strategic teeth 4. Replacement of missing upper anterior teeth with a removable prosthesis 5. Dental hygiene treatment that involves nonsurgical periodontal care to treat periodontal disease • Referral to off-center facilities as necessary for emergent or urgent conditions treatable beyond the expertise of general dentist C. Oral disease prevention education and management, to include: • Oral strategies, such as oral hygiene instruction, risk assessments, and group education. • Oral health promotion activities with an emphasis on overall wellness and employability. D. Providing a written diagnosis and treatment plan as part of an oral health and wellness plan for each student and obtaining agreement for voluntary care before proceeding with treatment. E. Providing oral health education. F. Establishing an appropriate referral system to ensure the receipt of specialty care as defined in the PRH and within budget constraints. G. Providing or arranging for 24-hour emergency coverage. H. Participating in the coordination and integration of the oral health program with the wellness components, Center activities, and Center community activities. I. Maintaining the accuracy and confidentiality of all required oral health and wellness records when they are in the subcontractor’s presence. J. Providing support to the students in acquiring and practicing the oral health-related skills, knowledge, and attitudes that will make them employable. K. Developing and complying with Job Corps infection control policies and procedures. L. Reviewing and signing Health Care Guidelines annually for dental care in accordance with PRH requirements. M. Enforcing appropriate student workplace behavior when students are in the dental office or waiting area. N. Arranging appropriate medial separations in conformity with the PRH. O. Collecting data and preparing reports as required by the Center Director and/or the Department of Labor. P. Advising/consulting as requested with the Center Director and Center staff on oral health and dental programmatic issues. Q. Cooperating with corporate and Regional Office Center assessments. R. Ensuring that regular preventive maintenance is provided to the dental equipment. The Center Dental Assistant agrees to adhere to the policies, procedures, and guidelines established by Job Corps. It is understood and agreed that the services provided by the Center Hygienist are subject to monitoring and review by the Department of Labor. III. BID REQUIREMENTS 1. Final Bid Proposal shall be typed and submitted on official Company letterhead and must include the following: • Scope of Work ID# • References • Copies of all Insurance Certificates • Copy of all licenses and certifications relevant to this proposal • Completed Vendor Questionnaire (with DUNS# if available) and W9 • Small Business Certification, if any Submitted bid will show at minimum breakdown as follows: • Price per hour • Please include any additional fees Period of Performance is from 12/1/15-11/30/16. If unable to quote, please indicated ‘No Bid’ and return bid package to us. Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly. Bidders are placed on notice that this is a competitive bid process and cost is an important factor. Overall low bid does not constitute bid award. Atterbury Job corps Center is a federally funded facility. Payment terms are Net 30 days from date of invoice. Vendor is required to show number of hours billed and hourly rate on invoice, list of materials used to complete service and provide certified payroll records with invoice. All questions regarding the Invitation to Bid and/or scope of work must be submitted in writing to: chester.karla@jobcorps.org. No phone calls will be accepted. Vendor shall hold all licenses necessary to do business in the state of Indiana and provide copies of all licenses with bid package. Vendor shall obtain and maintain insurance as follows: • Workers Compensation must meet state statutory limit requirements, including those for Employer’s Liability, if applicable • Comprehensive General Liability with Atterbury Job Corps Center listed as additional insured. ($1,000,000 per occurrence). • Comprehensive Automobile Liability ($1,000,000/combined single limit) • Fire legal Liability ($5,000,000 per occurrence) • The proposed shall cover any and all expenses not covered by said insurance including all deductibles. Bids will be due by Monday, September 28th, at 3:00 pm East Coast Time. IV. EVALUATION CRITERIA Evaluation Criteria Point Value Price: Responsible Contractor who can perform effective services at the lowest reasonable price. 60 Quality: Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints. 20 Past Performance: Past services to center or list of references for verification purposes. 10 Schedule: Explanation on how services will be carried out and the delivery timeframe. 10 TOTAL POINTS 100 Award will be made to the offeror who provides Best Value to the Center when all Evaluation Criteria are considered. Lowest Price does not guarantee award of Sub Contract. Adams and Associates retains the right to reject any and all bids.
