- Standard Form LLL - Please fill out regardless if you lobby or not. Site Map for Fera United States Army Reserve Center Solicitation INSTRUCTION TO OFFERORS 1. THIS IS A SYNPOSIS/SOLICITATION IN ACCORDANCE WITH FAR PART 12 All questions shall be in writing. Questions regarding this solicitation shall be submitted via email no later than 11:00 AM EST on 18 September 2015 to Captain Eugene Choi, Contract Specialist, at Eugene.choi3.mil@mail.mil AND Sharon Wilson-Emmons, Contracting Officer, at sharon.wilson-emmons.civ@mail.mil 2. All proposals are due no later than the time specified in Block 8 of the SF1449 Eastern Standard Time. Proposals must be emailed to Captain Eugene Choi at eugene.choi3.mil@mail.mil 3. You must include all representations and certifications that are included in this solicitation with your proposal. 4. It is the offeror's responsibility that all proposals submitted, regardless of method of transmission, are received in this office prior to the time and date for receipt of proposal. 5. Anywhere in this solicitation where the term "quote" is used, it is hereby changed to read "offer". 6. Anywhere in this solicitation where the term "contractor" is used, it is hereby changed to read "offeror". 7. You must be registered in the System for Award Management (SAM) in order to be awarded a Department of Defense contract. If you need to register in SAM, go to the website https://www.sam.gov/sam 8. Due to restricted access to Fort Dix, contractors that will be hand carrying their proposal are required to submit tothe contracting officer a list of personnel planning to deliver the package not later than 24 hours prior to due date for receipt of proposal. Contractors must also furnish the approximate time of arrival and the gate they will utilize. Contractors are cautioned that there may be delays at the gates and should allow adequate time for entering the installation to ensure packages are received prior to the time and date for receipt of proposal. All individuals must have picture identification and no substitutions are authorized. Vehicles are also subject to search. 9. All work is to be completed in accordance with the Performance Work Statement. 10. The Government intends to award a Firm Fixed Price (FFP) contract. The Period of Performance for this effort shall be a twelve month base year and four, twelve month option years from the end of the base year. 11. See Federal Acquisition Regulation (FAR) Clause 52.212-1 "Instructions to Offerors - Commercial Items" and FAR 52.212-1, Addendum for additional information. 12. See FAR 52.212-2, Addendum for evaluation criteria. 13. All offerors who propose to utilize subcontractors shall submit the name, address and the proposed percentage of the work to be performed for each subcontractor. Failure to comply may deem the offeror noncompliant. 14. All offers shall remain valid for 120 calendar days. 15. Attachments (Site Maps): 1. FERA USAR CENTER SITE MAP 2.
